A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO CITY OF CHICAGO FOOD SANITATION CERTIFICATES WERE POSTED ON THE PREMISES AT TIME OF INSPECTION. INSTRUCTED MANAGEMENT THAT A CERTIFIED FOOD MANAGER MUST BE PRESENT WITH THEIR ORIGINAL CERTIFICATE POSTED ON PREMISES WHILE POTENTIALLY HAZARDOUS FOODS ARE PREPARED OR SERVED. PREMISES SERVES HOT BREAKFAST, LUNCH AND DINNER TO RESIDENTS MONDAY THRU SUNDAY. SERIOUS VIOLATION 7-38-012.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. NO LIGHT SHIELD OBSERVED OVER THE STOVE IN THE PREP AREA. INSTRUCTED MANAGEMENT TO PROVIDE.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. OBSERVED CIGARETTE BUTTS ON THE GROUND OUTSIDE OF THE REAR EXIT DOOR TO THE KITCHEN. MANAGEMENT INSTRUCTED TO CLEAN AND MAINTAIN THIS AREA.

All employees shall be required to use effective hair restraints to confine hair. ANYONE ENTERING IN KITCHEN MUST WEAR HAIR RESTRAINTS,INCLUDED DIETITIAN. ALSO ALL EMPLOYEE'S PERSONAL ITEMS MUST BE REMOVED FROM KITCHEN AND PREP AREA(COAT,HANDBAG,SHOES,SLIPPER ETC.)PROVIDE A LOCKER ROOM.
Adequate and convenient toilet facilities shall be provided. They should be properly designed, maintained, and accessible to employees at all times. STAFF WASHROOM NOT MAINTAINED,IN BASEMENT AREA. WE(MANAGER ,ENGINEER AND MYSELF)OBSERVED NO PAPER TOWELS PROVIDED IN MEN'S WASHROOM.ENGINEER STATED THAT ARE AWAITING FOR THE DELIVERY,NO PAPER TOWEL SINCE 1-25-12.AFTER I SPOKE WITH ADMINISTRATE AFTER WHILE PAPER TOWELS WAS PROVIDED. CRITICAL VIOLATION:7-38-030 HOOOO77341-15

All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OUTER OPENINGS NOT PROTECTED. OBSERVED REAR EXIT DOOR WITH A 2'GAP ON SIDE AND BOTTOM OF THE DOOR. DOOR AND FRAME ARE RUSTED. ALSO OBSERVED THE DOORS IN DINING ROOM THAT CONNECTS THE PATIO ARE EXTREMELY RUSTED AND THE TOP OF THE FRAME WITH HOLES LARGE ENOUGH FOR INSECT ENTRY. WHEN YOU OPEN THE SAID DOORS RUST FALLS ON YOU AND FLOOR. MANAGEMENT IS REQUIRED TO REPLACE DOOR FRAMES. REAR DOOR MUST BE RODENT PROOFED.SERIOUS VIOLATION:7-38-020 HOOO077342-16
The walls and ceilings shall be in good repair and easily cleaned. REMOVE DIRTY AND UNEVEN CAULKING FROM BEHIND THE 2 COMPARTMENT SINK,REMOVE AND REPLACE(NORTH-WEST SIDE).
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REPAIR LEAKING MECHANICAL DRAIN STOPPER INSIDE THE THIRD COMPARTMENT SINK,EMPLOYEE ARE USING A CLOTH TO SEAL COMPARTMENT SINK. FAUCET IS LEAKING AT 3 COMPARTMENT SINK, REPAIR IT.
