EMPLOYEE BELONGINGS MUST NOT BE STORED IN FOOD/BAR AREAS. OPEN OUTDOOR SHED (NO WALLS) FOUND STORED WITH OLD EQUIPTMENT,OLD PLUMBING ITEMS,TOOLS,VARIOUS OTHER ITEMS STORED FROM FLOOR TO CEILING. MUST BE REMOVED FROM PREMISES TO PREVENT RODENT HARBORAGE. ALL ARTICLES STORED MUST BE ENCLOSED WITH TIGHT FITTING ENTRANCE/EXITS AND ITEMS STORED ELEVATED AND ORGANIZED.
All hot food shall be stored at a temperature of 140F or higher. OBSERVED PREVIOUSLY PREPARED MASHED POTATOES BEING REHEATED IN STEAMTABLE. POTATOES FOUND AT 125.7F. APPROXIMATLY 5LBS DISCARDED AND INSTRUCTED MUST RAPIDLY REHEAT PREPARED FOODS TO 165F THEN TO HOLD HOT ONLY ON STEAMTABLE.
All food shall be protected from contamination and the elements, and so shall all food equipment, containers, utensils, food contact surfaces and devices, and vehicles. OBSERVED BOTH ICE MACHINES ON PREMISES WITH ICE (FOOD) NOT PROTECTED FROM CONTAMINATION. UPPER PANEL OF BOTH MACHINES WHERE WATER RESERVIOR MAKES ICE AND THROUGHOUT BOTTOM BIN WHERE ICE IS STORED FOUND WITH EXCESSIVE BLACK JELLIE LIKE SUBSTANCE TOUCHING AND DRIPPING INTO ICE AND ON ALL ICE CONTACT SURFACES.ALL ICE DISCARDED. BOTH MACHINES TAGGED HELD FOR INSPECTION AND MUST NOT USE UNTIL CLEANED, SANITIZED AND SUPERVISOR SALAZAR NOTIFIED @312-746-8030 AND LETTER FAXED TO C.D.P.H. TO REQUEST INSPECTION AND TAG REMOVAL.FAX #312-746-4240.

WEST EXIT DOOR TO PARKING LOT FROM 'MAIN CLUB' NOT RODENT PROOFED WITH OPEN 1/4 TO 1/2 INCH GAP ON BOTTOM AND TORN EXTERIOR WEATHER/RODENT STRIP ON BOTTOM. INSTRUCTED TO PROVIDE A TIGHT FITTING SEAL ON DOOR.

OUTSIDE ROLL OFF WASTE RECEPTACLE WITH OVERFLOWING BAGS OF GARBAGE AND DEBRIS. MISSING AND BROKEN LIDS UNABLE TO HAVE A TIGHT FITTING SEAL. BAGS OF GARBAGE ON GROUND WITH PILED WOOD, PAPER DEBRIS, EMPTY WINE BOTTLES, DISCARDED CAR SEAT AND OTHER ASSORTED ARTICLES ALL ON SURROUNDING GROUND AROUND CONTAINER. ALSO OBSERVED OPEN PLASTIC GARBAGE CANS FILLED WITH FOOD DEBRIS AND EMPTY LIQUOR BOTTLES OUTSIDE NEXT TO PARKING LOT EXIT DOOR AND OPEN BOX OF EMPTY LIQUOR BOTTLES AND PLASTIC CUPS ON GROUND NEXT TO BUILDING ALONG REAR WALKWAY TO REAR KITCHEN ENTRANCE. INSTRUCTED MUST REMOVE ALL OPEN CONTAINERS OF GARBAGE. REMOVE OVERFLOWING GARBAGE AND PROVIDE TIGHT FITTING LIDS FOR CONTAINER. REMOVE ALL DEBRIS FROM GROUNDS OF SURROUNDING AREA AT OUTSIDE WASTE RECPTACLE.
All food not stored in the original container shall be stored in properly labeled containers. BULK FOOD CONTAINERS MUST BE LABLED.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST INSTALL SPLASH GUARD AT REAR PREP EXPOSED HAND SINK RIGHT SIDE TO PREVENT CROSS CONTAMINATION ON STORED CLEAN DISHWARE ON SHELVING.

Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. 2ND FLOOR EMPLOYEE WASHROOM MUST HAVE SELF-CLOSING DEVICE. ALSO EMPLOYEE WASHROOM DOOR IN LOCKER ROOM MUST BE ABLE TO SELF CLOSE.
