APPROXIMATELY 20 LIVE FRUIT FLIES WERE OBSERVED ON THE CLUTTERED NORTH WALL AROUND THE WATER MAIN PIPING IN BASEMENT, REAR DRY STORAGE ROOMS ALSO AT BAR AREAS 1ST,2ND FLOOR .ALSO REAR SIDE DOOR TO PREMISES,DOOR TO OUTSIDE PATIO,TO THE SMOKER ROOM NOT INSECT/RODENT PROOF 1/2' MODE OF ENTRY ALSO WINDOW FRAME ABOVE REAR EXIT DOOR HAS OPENING ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY,INSECT/RODENT PROOF SAID DOORS WINDOW FRAME. SERIOUS VIOLATION 7-38-020.

THERMOMETERS ARE NEEDED AND MAINTAINED INSIDE ALL REFRIGERATION UNITS.
Food handlers must have hair restraints when working around open food.
Ice scoops must be stored in clean dry place. Wiping cloths must be stored in sanitizing solution.
