All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. 3COMP. SINK WALLS AND TOILET BOWL INSIDE WITH BUILD UP OF DIRT NEEDS CLEANING.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. MUST ELEVATE BOXES OF SINGLE SERVICE ARTICLES STORED ON FLOORS AT REAR 6' OFF FLOOR.
All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. MUST PROVIDE SHIELD ON LIGHT BULB ABOVE 3COMP.SINK & SHELVES.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REPAIR PIPE LEAKING UNDER EXPOSE HAND WASH SINK IN KITCHEN,AND REPAIR FAUCET LEAKING WATER AT 3COMP.SINK. MUST ALSO REMOVE POTERY SEAL INSIDE ABOVE SAID HAND WASH SINK AND WELD INSTEAD.

INSTRUCTED TO REPLACE MISSING SINK STOPPER IN THE MIDDLE SINK OF 3COMP. MUST REPLACE MISSING FILTER PIECE ON HOOD,REPAIR RUSTED SHELVES IN WALK IN COOLER,AND REMOVE CARD BOARD LININGS ON WALK-IN COOLER SHELVES.

Dish washing facilities not maintained or operated. While running and draining water in middle of three compartment sink, water coming up left and right sides if sink-water cloudy with food debris. Instructed to detail clean drainline and grease trap to prevent inadequate disposal of waste water. Serious citation issued 7-38-030.
