Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO REPAIR LEAKY DRAINPIPE UNDER 3 COMPARTMENT SINK.------NOTED HOT AND COLD WATER FAUCET CONTINUOUSLY DRIPPING ON FLOOR IN THE BASEMENT NEXT TO MOP SINK, MUST FIX.--------INSTRUCTED TO PROVIDE 3 WORKING DRAIN STOPPERS AT 3 COMPARTMENT SINK.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. INSTRUCTED TO PROVIDE THERMOMETER VISIBLE AND ACCURATE INSIDE ALL COOLER AND FREEZER.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. INSTRUCTED TO CLEAN AND REMOVE UNNECESSARY ARTICLES STORED IN REAR ROOM OF THE BASEMENT STORAGE TO PREVENT RODENT AND INSECT HARBORAGE.
The walls and ceilings shall be in good repair and easily cleaned. Use dustless cleaning methods. INSTRUCTED TO CLEAN AND MAINTAIN CEILING VENT COVER WITH HEAVY DUST BUILD-UP IN BASEMENT TOILET ROOM FOR PROPER AIR FLOW.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. FOUND NO CERTIFIED FOOD MANAGER ON DUTY WHILE POTENTIALLY HAZARDOUS FOODS (EX TACOS, REFRIED BEANS, BARBACOA, TAQUITOS, RICE ETC.) ARE BEING PREPARED AND SERVED. INSTRUCTED PERSON IN CHARGE, A CERTIFIED FOOD MANAGER WITH CITY OF CHICAGO FOOD SERVICE SANITATION CERTIFICATE MUST BE PRESENT WHEN POTENTIALLY HAZARDOUS FOODS ARE BEING PREPARED AND SERVED. INSTRUCTED TO DETAIL CLEAN FLOORS WITH DRIED FOOD DEBRIS ACCUMULATION ALONG THE WALLS AND IN ALL CORNERS AND FLOORS IN BASEMENT.
INSTRUCTED TO INSTALL BACK FLOW PREVENTER DEVICE/VACUUM BREAKER AT MOP SINK FAUCET IN THE BASEMENT.------------MUST INSTALL EXPOSED HAND SINK IN THE BASEMENT (NOTED PREP TABLE AND EQUIPMENT) OR REMOVE PREP TABLE AND EQUIPMENT.
LIGHT MUST BE SHIELDED ABOVE PREP TABLE IN THE BASEMENT OR PROVIDE SHATTER PROOFED LIGHT BULB.
INSTRUCTED TO REPAIR BROKEN SELF CLOSING DEVICE AT WASHROOM DOOR IN THE BASEMENT.
