All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INSTRUCTED TO CLEAN AND MAINTAIN THE FOLLOWING: HOOD AND FILTERS WITH GREASE BUILD-UP ABOVE COOKING EQUIPMENT IN TAQUERIA AREA, IN BETWEEN RUBBER GASKET WITH DRIED FOOD DEBRIS AROUND PREP COOLER DOOR (TAQUERIA SIDE), EXTERIOR SURFACES OF GARBAGE BINS, LIGHT SHIELDS WITH GREASE BUILD-UP ABOVE COOKING/TAQUERIA AREA AND DISPLAY RACKS INSIDE DAIRY DISPLAY COOLER.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. INSTRUCTED TO CLEAN AND MAINTAIN FLOORS ALONG THE WALLS AND IN ALL CORNERS THROUGHOUT REAR STORAGE AREA, UNDER AND BEHIND COOLERS AND FREEZERS.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. LIGHTS MUST BE SHIELDED INSIDE MEAT WALK-IN COOLER OR PROVIDE SHATTER-PROOFED LIGHT BULBS.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. INSTRUCTED TO CLEAN AND REMOVE UNNECESSARY ARTICLES STORED ON TOP OF ALL WALK-IN COOLER (DAIRY AND MEAT) TO PREVENT RODENT AND INSECT HARBORAGE.-------MUST REORGANIZE REAR STORAGE AREA AND REMOVE UNUSED ITEMS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO PROVIDE WORKING DRAIN STOPPERS AT (3) 3 COMPARTMENT SINK.--------NOTED 3 COMPARTMENT SINK BEHIND MEAT PREP/DISPLAY AREA IN POOR REPAIR WITH LEAKY FAUCET NECK. INSTRUCTED TO FIX.
The walls and ceilings shall be in good repair and easily cleaned. INSTRUCTED TO CLEAN WALLS WITH STAIN AND SPILLS BEHIND EXPOSED HAND SINK AND MOP SINK IN REAR AREA.-------MUST CLEAN WALLS AND COUNTER SHELVES IN TAQUERIA AREA.

Dishwashong facilities not maintained or operated. While draining water in middle of three compartment sinkfront prep area, water-greasy with food debris coming up left and right sides of sink. Instructed to detail clean grease trap and drainline to prevent inadequate disposal of waste water. Serious citation issued 7-38-030.
Evidence of mice droppings on premises. Observed approximately 20 mice dropping in north east corner where clutter is stored inside boxes and approximatley 10 mice dropping inside boiler room storage shelves right side at bottom. Instructed to remove all activity and detail clean areas. Serious citation issued 7-38-020.
Floor under display shelves and along wallbase in rear storage areas not cleaned. Instructed to detail clean daily.
Exposed handsink installed next to other sinks in prep areas. Instructed to install shield on both sides of exposed handsink.
Sinks, coolers shelves, exhaust hood and filters, fan covers in walk-in cooler, cooking equipment not cleaned. Instructed to detail clean and sanitize.
Ceiling and ventilation cover in butcher area not maintained. Instructed to detail clean vent cover and replace worn ceiling tile. Outer front dual doors worn in middle. Instructed to seal doors in middle.
Light bulbs not shielded under produce display case. Instructed to install shields and end caps.
Unnecessary articles stored in rear of premises. Instructed to remove all unused articles and elevate stock 6 inches or higher and away from wall.
