Pest control logbook not provided or mantianed on premises. Instructed to obtain a licensed pest control oompany and service premises and provide logbook with all required documents. Front entry door not rodent proofed. Instructed to to rodent proof front door to prevent rodent and insect entry. Instructed manager to install screen door to prevent flying insects or keep door closed during operation.
Water pressure low at all sinks. Instructed to increase water pressure under city pressure. Taylor soft serve machine not provided with hot and cold running water supply at top of machine. Instructed to install hot and cold running water.
Coolers not maintaining proper temperature. Instructed to have all coolers on and running at 40 F or below.

Missing light shields in rear kitchen area must be provided, light shields in kitchen prep area need cleaning.
Exhaust canopy not clean had excess grease build up, needs detailed cleaning.
