All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. INADEQUATE PEST CONTROL-FOUND APPROXIMATELY 50 LIVE FRUIT FLIES IN VCARIOUS AREAS OF KITCHEN,BAR,DISHMACHINE ROOM,FOLLOWING DOORS NOT RODENT-PROOFED WITH 1/4' MODE OF ENTRY FRONT SOUTH DOORS,REAR EXIT DOOR LEADING TO GARBAGE DUMPSTERS.DETAIL CLEAN,SANITIZE ALL AFFECTED AREAS,REMOVE MENTIONED PEST,PEST CONTROL SERVICE IS RECCOMENDED,RODENT PROOF SAID DOORS.SERIOUS VIOLATION 7-38-020.
All food not stored in the original container shall be stored in properly labeled containers.ALL BULK FOOD CONTAINERS MUST BE LABELED TO IDENTIFY PRODUCTS.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. ICE BIN COVERS ARE NEEDED FOR ICE BINS AT BAR AREAS.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN FAN COVERS INSIDE THE WALK-IN-COOLER,SHELVINGS,ALONG KITCHEN/DISHMACHINE/BAR AREAS,MANUAL CAN OPENER.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS ARE DIRTY AT BAR AREAS,CLEAN FLOORS IN DETAIL.
The walls and ceilings shall be in good repair and easily cleaned. DETAIL CLEAN WALLS/CEILINGS ALONG KITCHEN,DISHMACHINE AREAS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. DRIP TRAY AT BAR AREA NOT PROPERLY CONNECTED MUST CORRECT.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units.THERMOMETERS ARE NEEDED INSIDE ALL REFRIGERATION UNITS.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors.PERSONAL BELONGINGS STORED IN FOOD PREP AREA,DISHMACHINE AREAS MUST BE PROPERLY STORED IN A DESIGNATED AREA,MILK CRATES MUST NOT BE USED FOR STORING OF ARTICLES.
All employees shall be required to use effective hair restraints to confine hair.MUST PROVIDE HAIR RESTRAINTS FOR ALL FOOD HANDLERS.

Previous serious violation not corrected:3-6-12, report # 170193: #18-Front and rear doors not rodent proofed. front double doors has 1/4 inch gap in middle. Instructed to rodent proof to prevent rodent and insect entry. Live gnats activity has been minimized through-out premises. Critical citation issued 7-42-090. Manager has temporarily repaired door.
Floors are not clean in corner of walk-in cooler.
Condensation line not properly connected to floor drain.
