The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Reseal the rear kitchen floor grout so the floors are smooth, sanitary and easy to maintain.
The walls and ceilings shall be in good repair and easily cleaned. Clean the rear kitchen ceilings and maintain them in sanitary condition.

Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. SOME SLEEVE'S OF CUPS AND LIDS STORED UNDER THE SOUP STATION FOUND IN THE DIRTY CABINET INTERIOR WHERE DEAD ROACHES WERE FOUND WERE VOLUNTARILY DISCARDED AT THIS TIME BY MANAGEMENT. STORE SINGLE SERVICE ARTICLES IN A SANITARY MANNER.

DIRTY EQUIPMENT CASTERS AND WHEELS AND THE CONDENSER WATER CATCH PAN UNDER THE MILK SHAKE COOLER IN THE FRONT PREP AREA NEED TO BE CLEANED TO REDUCE THE POSSIBLE COCKROACH HARBORAGE THEY POSE.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. SEAL UP ALL GAPS AND CREVICES WHERE DEAD GERMAN COCKROACHES WERE FOUND UNDER THE HOT SOUP WELLS IN THE CABINETS TO REDUCE INSECT HARBORAGE, ANDERSON PEST CONTROL IS MONITORING SAID AREA AND LAST SERVICED SAID AREA ON 4/21/11 PER REPORT. REPAIR THE WALK IN COOLER DOOR SO IT SELF CLOSES.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.MUST REPAIR LEAKY FAUCET AT HANDSINK IN FRONT PREP AREA AS WELL AS LEAK UNDER DIPPER WELL IN FRONT OF ICE CREAM FREEZER.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors.MUST BETTER ORGANIZE BASEMENT STORAGE AREAS AND ITEMS MUST BE 6 INCHES OFF THE FLOOR AT ALL TIMES.

MUST CLEAN INTERIOR OF ALL SERVICE COOLERS AND WALK IN COOLER TO REMOVE SPILLS AND/OR DEBRIS.
