The licensee shall inform the public in writing of the Chicago Department of Public Health?s Consumer Advisory relating to consuming raw and undercooked animal products.OBSERVED PACKAGED FISH IN REACH IN COOLER IMPROPERLY LABELED INSTRUCTED ON PROPER LABELS NAME OF PRODUCT,NAME OF BUSINESS, ADDRESS, PHONE #, ETC.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.OBSERVED THE FOLLOWING NOT CLEAN AND SANITIZED CUTTING BOARDS, AND CLEAN THE FOLLOWING INTERIOR OF 3 DOOR COOLER, FAN GUARD COVER, MICROWAVE , STORAGE SHELVES.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.OBSERVED RAW WOOD AROUND FISH TANK, BEHIND 3 COMPARTMENT, REAR DOOR, TABLE STORAGE SHELF INSTRUCTED TO SEAL / PAINT ALL RAW FISH.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.OBSERVED THE FRONT FLOOR TILE JOB NOT FINISHED INSTRUCTED TO FINISH, CLEAN FLOOR THRU OUT FRONT REAR AND BASEMENT.
