ALL FLOORS NEEDS TO BE DETAIL CLEANING, CLEAN DROPPING IN THE ACTIVE ROOM. ALSO REMOVE ALL THE CLUTTER IN THE STORAGE ROOM, EVERY THING NEESD TO BE ELAVATED 6 INCH OFF THE FLOOR AND 6 INCH AWAY FORM THE WALLS

OF HEALTH. INST NEEDS TO HAVE ALTERNATIVE FOOD SOURCE. PREMISE IS PLACED ON 48HR NOTICE.

FOUND THE REAR DOOR IN THE KITCHEN AREA NOT RODENT OR INSECT PROOFR, 1/4 IN GAP OPENING,THE 2ND DOOR LEADING TO THE PARKING LOT OPEN GAP 1/4 INCH GAP, FRONT DOOR OPEN GAP 1/4 GAP. FOUND MICE DROPPINGS IN THE ACTIVE ROOM IN THE CLOSET AREA 10-20 DROPPING TO CLUTTER NO ABLE TO SEE IN THE CORNER, NOTE FOUND DROPPING IN THE BASEMENT AREA IN ALL CORNERS. NEEDS TO CLEAN AND GET THE PEST CONTROL TO RE-EVULATE THE PROBLEM. NEEDS TO HAVE A MAP WITH ALL THE INFORMATION.
