EXPOSED HANDSINK WAS NOT CONNECTED; PIPE CAME LOOSE AND EMPLOYEE STATED THAT IT WAS GOING TO BE REPLACED. WHEN MANAGER ARRIVED HE HAD SOMEONE REPAIR SINK WHILE I WAS HERE. NO OTHER HANDSINKS ON PREMISES AND THIS ONE IS LOCATED OUTSIDE OF WASHROOM IN FOOD PREP AREA.
A LIVE CAT WAS OBSERVED LIVING IN BASEMENT. LITTER BOX WITH FECES AND FOOD SEEN ON FLOOR FOR CAT. INSTRUCTED THAT NO LIVE ANIMALS CAN BE ON PREMISES AT ANY TIME. CAT WAS REMOVED.
NO CITY OF CHICAGO FOOD SERVICE MANAGER ON DUTY WITH A VALID CERTIFICATE WHILE POTENTIALLY HAZARDOUS FOODS WERE BEING PREPARED AND SERVED. FOOD TEMPS OF HOT FOODS WAS FOOD THAT EMPLOYEE STATED HE WAS PREPARING FOR HIMSELF AND OTHER WORKERS. CUSTOMERS ORDERED NACHO CHEESE WHILE I WAS ON PREMISES. MUST HAVE A CERTIFIED MANAGER ON DUTY AT ALL TIMES FOOD IS BEING PREPARED AND SERVED. COOK STATES THAT HE LEFT HIS CERTIFICATE AT HOME.

A LIVE CAT AND CAT'S LITTER BOX WAS OBSERVED IN BASEMENT. OWNER HAD EMPLOYEE PUT CAT OUTSIDE. INSTRUCTED OWNER THAT HE CANNOT UNDER ANY CIRCUMSTANCES HAVE A LIVE CAT ON PREMISES. ESTABLISHMENT WAS VIOLATED FOR THE SAME CAT ON PREMISES ON NOV. 21, 2012.
NO CERTIFIED MANAGER ON DUTY AS REQUIRED. COOK ON PREMISES STATES THAT HIS CERTIFICATE FELL BEHIND THE DEEP FRYER WHILE HE WAS CLEANING THE WALLS. INSTRUCTED THAT THE CERTIFICATE MUST BE POSTED AND DISPLAYED AT ALL TIMES. ADVISED TO REMOVE ITEMS FROM WALLS BEFORE CLEANING THEM. COOK TRIED TO RETRIEVE CERTIFICATE FROM FLOOR; IT TORE. CERTIFICATE WAS A XEROX COPY AND INFORMATION NEEDED FROM CERTIFICATE WAS STILL NOT OBTAINED. WHILE TYPING REPORT, COOK WENT HOME TO GET ORIGINAL CERTIFICATE AND RETURNED WITH NOTHING. INSTRUCTED TO CEIST AND DESIST COOKING IN RESTAURANT AREA OF STORE AT ONCE.
