A LIVE CAT WAS OBSERVED LIVING IN BASEMENT. LITTER BOX WITH FECES AND FOOD SEEN ON FLOOR FOR CAT. INSTRUCTED THAT NO LIVE ANIMALS CAN BE ON PREMISES AT ANY TIME. CAT WAS REMOVED.
NO CITY OF CHICAGO FOOD SERVICE MANAGER ON DUTY WITH A VALID CERTIFICATE WHILE POTENTIALLY HAZARDOUS FOODS WERE BEING PREPARED AND SERVED. FOOD TEMPS OF HOT FOODS WAS FOOD THAT EMPLOYEE STATED HE WAS PREPARING FOR HIMSELF AND OTHER WORKERS. CUSTOMERS ORDERED NACHO CHEESE WHILE I WAS ON PREMISES. MUST HAVE A CERTIFIED MANAGER ON DUTY AT ALL TIMES FOOD IS BEING PREPARED AND SERVED. COOK STATES THAT HE LEFT HIS CERTIFICATE AT HOME.
