Outer openings not protected or rodent proofed and evidence of rodent and live insect activity. Found approximately 25 plus live gnats on walls leading to receiving area and approximately 30 plus mice droppings inside walk-in cooler used for dry storage. Also, north, south and west exit doors has 1/4 inch gap across bottom. Instructed to rodent proof doors to prevent rodent and insect entry and remove all pest activity and sanitize areas. Serious citation issued 7-38-020.
Outside garbage(1143)area not cleaned, maintained or rodent proofed. Observed to garbage dumpster overflowing with garbage and debris. Dumpster lids are elevated and tight fitting to prevent rodent or insect entry. Instructed to have dumpster lids tight fitting to prevent rodent and insect nuisance. Serious citation issued 7-38-020
Facilities not maintaining proper temperature. Found 2 door reach-in cooler at 54.0 F while storing potentially hazardous foods. Instructed that coolers must maintain 40 F or below. 'Held for Inspection' tag placed on cooler. Critical citation issued 7-38-005(A).
Potentially hazardous foods does not meet temeprature requirements during storage, preparation, display and service. Found 50 lbs of Liquid eggs at 43.6 F to 45.9 F, value $75. Bacon 30 lbs at 57.0 F, value $124. Sausage 10 lbs at 52.4 F, value $34. Also, dining room in 1143 building has salad bar with cold items sitting out at room temeprature. Found boiled eggs at 4 dozen 49.6 F, value $15. Instructed manager that potentially hazardous foods must maintain 40 F or below. Must provided cold holding unit or storage for salad bar. Critical citation issued 7-38-005(A). Total value $248.00, 94 lbs. Manager voulntarily denatured and discarded foods.

Gaskets not maintained on 2 door reach-in cooler. Instructed to replace worn gaskets.
Exhaust hood filters, interior of ice machine panel, deep fryer cabinets, and can opener attached to prep table not cleaned. instructed to detail clean and sanitize. Insterior of walk-in freezer has ice build-up. Instructed to defrost freezer and remove ice.
Leak noted under three compartment sink in main kitchen. Instructed to repair to prevent leak. Men's toilet room (1143)handsink in middle-cold not working. Instructed to repair and provided cold water.
Light covers in prep area not cleaned. Instructed to detail clean. Several prep area has chipped paint. Instructed to remove chipped paint and repaint walls.
Unnecessary articles in walk-in cooler for dry storage and 1143 boiler room. Instructed to remove all unused articles. detail clean areas and elevate supplies 6 inches or higher and away from walls.
Light bulb inside walk-in freezer not shielded. Instructed to install shatter-proof bulb.

There was a flooding in basement last week on July3,2023 at 1143 N Lavergne Ave Chicago, Il due from heavy rains the day before; as told by staff, janitor and fireman and also an electrical fire that had to be put out with an electrical outage. *All tenants had to evacuate. Transfered to one of their other corp.buildgs I am all way far out to Eagle's View 200 W International Ave Apt 216 Rantoul Il and near Chapmaign, Il nearby town in Champagne County. Springfield also next town. You not believe I sent with a vehicle full of others sent too far! I have er my concerns and specialist I see and it would be a 3 1/2 hr travel 1hr appt visit and 3 1/2 hr travel back. I not know I could endure an 8 hr day of that travel Do much and dr.appt. together. What you say, think?! I not know why they could not send us to a closer PLC.until repairs made and go back to apt. Where I am at people much nicer. *Do you know if repairs have been made as of yet and today's date 7/10/2023? The buildg.was closed up. First said 24 evacuation then 2 days then 3 and last week told tenants be returning this week with no update as of yet. If one call main phone number; only VM full on phone. Personally all my belongings still in my apt. And I was sent to Eagle's View Senior Living Apts. over 100 miles away from Chicago and my residence there at 1143 N Lavergne Ave Apt 318 Chicago, Il 61866. I am concerned of my belongings going back to or if I decide everything moved on where I am at. The staff here said they could get my belongings and bring here, though a sense of not know coming or going and if and when.
