OBSERVED FRUIT FLY ACTIVITY IN THE FOLLOWING AREAS: DISH AREA, REAR PREP, SUSHI BAR AND FRONT BAR. 20-25 FLIES NOTED. MUST CONTACT PEST CONTROL COMPANY FOR SERVICE. CLEAN ALL AREAS. SERIOUS VIOLATION 7-38-020 CITATION ISSUED. COURT APPEARENCE ON THURSDAY APRIL 29, 2010 @9:00 AM ROON 112 400 W SUPERIOR.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. CUTTING BOARDS IN NEED OF REPLACING. WORN WITH DEEP CUTS, STAINED. MUST REPLACE AND MAINTAIN.

HANDSINK FAUCETS ARE LEAKING AT THE COOK'S LINE, & SLOW-DRAINING 1-COMP. SINK AT THE BAR MUST BE FIXED. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.
ANY SODA OR MILK CRATES USED FOR DRY OR COLD STORAGE RACKS MUST BE REPLACED WITH REGULAR SHELVINGS. All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors.

IMPROPER UTENSILS SANTATATION. OBSERVED AN EMPLOYEE WASHING A PITCHER THAT IS USED IN FOOD PREPARATION AT PREP AREA HAND SINK, NO SANITIZER USED, INSTRUCTED MGR TO WASH, RINSE AND SANITIZE UTESILS IN 3-COMP SINK OR DISH WASHING MACHINE.
USING GROCERY SHOPPING PLASTIC BAGS FOR FOOD STOREAGE. INSTRUCTED MGR TO USE APPROVED FOOD STORAGE BAGE AND CONTAINERS.
