Observed accumilated food debris inside prep coolers rubber door gaskets. Must clean and maintain all unclean cooler gaskets. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Observed accumilated food/dirt build up inside prep areas floor drains. Must clean and maintain all floor drains. The walls and ceilings shall be in good repair and easily cleaned. Must detail clean and sanitize all unclean walls throughout 2nd floor prep area and maintain. Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. Observed 1 missing ceiling light cover above prep area chest freezer. Must provide and install cover. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Observed missing sink handles at bar sinks(1st floor). Must repair and maintain plumbing.
