All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Must fix splashguard at main kitchen handsink (lopsided).

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Floors behind ice machine/chest freezer in basement & under the bar area must be kept clean.
The walls and ceilings shall be in good repair and easily cleaned. Dusty fan guards in small coolers must be kept clean.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Must fix leaky water line under 2-comp sink in basement.

THE 2ND FLOOR BAR 3 COMPARTMENT SINK HAS A BENT DRAIN IN THE EAST COMPARTMENT, REPAIR THE DRAIN SO IT IS FLUSH AND DRAINS EASILY.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE NORTH KITCHEN BASEBOARDS HAVE OPEN GAPS THAT CATCH FOOD DEBRIS, CLEAN AND MAINTAIN THE FLOORS AND BASEBOARDS SO THEY ARE SMOOTH AND SANITARY FROM CORNER TO CORNER.
The walls and ceilings shall be in good repair and easily cleaned. THE CEILING EAST AND WEST OF THE KITCHEN HOOD HAS A HEAVY GREASE BUILDUP, CLEAN AND MAINTAIN THE WALLS AND CEILINGS SO THEY ARE SMOOTH AND SANITARY FROM TOP TO BOTTOM. REPLACE THE MISSING CEILING TILE IN THE KITCHEN.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE DOUGH MIXER IN THE BASEMENT IS DIRTY AND NEEDS TO BE MOVED AWAY FROM THE GREASE TRAP.
