All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. COMPRESSORS & FAN GUARDS OF MOST COOLERS, WITH DUST BUILD-UP MUST BE KEPT CLEAN.
The walls and ceilings shall be in good repair and easily cleaned. DUST BUILD-UP ON WALLS ABOVE DISHWASHING AREA, BY KITCHEN DOOR & ABOVE A PREP TABLE MUST BE REMOVED & KEPT CLEAN.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS AROUND MAIN DRAIN IN BASEMENT NEED DETAILED CLEANING.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. RUSTY WALK-IN COOLER DHELVES MUST BE RE-SEALED OR REPLACED.

THE WALK IN COOLER HAS INADEQUATE LIGHTING, INSTALL BETTER LIGHTING.
The walls and ceilings shall be in good repair and easily cleaned. CLEAN AND REPAINT THE DIRTY KITCHEN WALLS.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. THE GRILL HOOD FILTERS ARE FULL OF GREASE, CLEAN OUT THE FILTERS.
THE KITCHEN HAS A LEAKING COOLER CATCH PAN, REPAIR SAID LEAK.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.MUST CLEAN FLOORS AT MAIN KITCHEN AREA UNDER, AROUND AND BEHIND HEAVY KITCHEN EQUIPMENT. MUST REPAIT CRACKED FLOOR AT BASEMENT IN FRONT OF WALK-IN COOLER.
The walls and ceilings shall be in good repair and easily cleaned.MUST REPAIR AND SEAL HOLE AT BASEMENT CEILING IN FRONT OF DRY STORAGE ENTRANCE SO AS TO BE SMOOTH AND EASILY CLEANABLE. MUST REPAIR/REPLACE WALL COUVINGS ALONG ALL BASEMENT WALLS NEXT HOT WATER HEATER AN IN FRONT OF WALK-IN COOLER.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.MUST REPAIR VENT AT LADIES RESTROOM ON THE 1ST FLOOR.
All employees shall be required to use effective hair restraints to confine hair.MUST PROVIDE HAIR RESTRAINTS FOR ALL FOOD PREP STAFF TO CONFINE HAIR AT ALL TIMES.
