Floor in corners and behind heavy equipment not cleaned. Instructed to detail. Rear exit door not completely sealed at bottom daylight coming through. Instructed to install treshold or install door sweep at bottom to seal.
Sinks, prep counter, cooking equipment and storage shelves inside cooler not cleaned. Instructed to detail clean and sanitize.
Thermometer not provided inside front cooler. Instructed to provide thermometer insude cooler visible.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOOR INSIDE WALK IN FREEZER UNCLEAN MUST CLEAN AND MAINTAIN
The walls and ceilings shall be in good repair and easily cleaned. WALLS BEHIND COOKING EQUIPMENT,BEHIND ICE CREAM MACHINE UNCLEAN MUST CLEAN AND MAINTAIN; DAMAGED WALLS UNDER 3 COMPARTMENT SINK AND UNDER 2 COMPARTMENT SINK MUST BE REPLACED,MUT PROPERLY SEAL WALLS

BURNED OUT LIGHT BULB ON THE HOOD, MUST REPLACE. MUST CLEAN THE LIGHT SHIELD ON THE HOOD,( WITH GREASE BUILD -UP) AND MAINTAIN.
MUST PROVIDE MISSING VISIBLE THERMOMETER INSIDE THE WALK IN COOLER AND WALK IN FREEZER.
MUST CLEAN IN DETAIL THE WALLS BEHIND THE 3 COMPARTMENT SINK. MUST SCRAPE PEELING PAINT ON THE WALLS BEHIND THE 3 COMPARTMENT SINK AND REPAINT.
MUST CLEAN IN DETAIL THE INTERIOR BOTTOM SHELVE OF THE PREP COOLER, AND REACH IN COOLER ( WITH FOOD CRUMBS)AND MAINTAIN.
ICE SCOOP MUST BE STORED IN A SEPARATE CONTAINER TO PREVENT CONTAMINATION(NOT ON TOP OF THE ICE). EMPLOYEE HANDLING FOODS IN THE PREP AREA MUST WEAR A HAIR RESTRAINT TO CONFINE HAIR.
FOUND ASHTRAY WITH CIGARETTE BUTTS INSIDE THE OFFICE OF THE MANAGER. INSTRUCTED MANAGER THAT ALL ASHTRAYS MUST BE REMOVED WHERE SMOKING IS PROHIBITED. CITATION ISSUED CLEAN INDOOR AIR ORDINANCE 7-32-020C.
