Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO PROVIDE WORKING DRAIN STOPPERS AT 3 COMPARTMENT SINK IN PREP AREA.----- INSTRUCTED TO PROVIDE VENTILATION INSIDE WASHROOM.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. INSTRUCTED TO PROVIDE THERMOMETER VISIBLE AND ACCURATE IN ALL COOLERS AND FREEZERS.

INSTRUCTED TO PROVIDE BACKFLOW PREVENTER DEVICE OR VACUUM BREAKER AT 2 COMPARTMENT SINK (USED AS MOPSINK) IN THE BASEMENT.

Facilities not maintaining proper temperature. Internal temperature of reach in cooler at 50.4 F while storing potentially hazardous foods. Instructed that cooler must maintain 40 F or below at all times of operation. Critical citation issued 7-38-005(A). 'Held for Inspection' tag placed on cooler.
Potentially hazarodus foods does not meet temperature requirement during storage, preparation, display and service. Found 3-30 Lbs of cooked Beef at 51.6 F and 4-30 Lbs of cooked pork at 49.9 F to 54.4 F and cooked rice 3 Lbs at 63.6 F in cooler stated above. Instructed manager that potentially hazardous foods must maintain 40 F or below. Critical citation issued 7-38-005(A). Manager voluntarily discarded and denatured. Value $600, 213 Lbs.
Light and ceiling tile not maintained. Instructed to replace all damaged ceiling tile and detail clean light covers through-out. Coving missing from front prep area along wallbase. Instructed to replace missing coving.
Foods inside coolers and bulk storage containers not labeled or dated. Instructed to label and date all foods not in original containers.
