All employees must wear garments that are clean and of washable character and nature and street clothes must be covered with an apron or sanitary uniforms used. All food handling employees shall be required to use effective hair restraints to confine hair.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. The metal stem thermometer is 8 f degrees off and was calibrated in front of the cook.

Only such poisonous and toxic materials as are required to maintain sanitary conditions may be used in food establishments and they shall not be used or stored in any hazardous manner. A SPRAY BOTTLE OF HEAVY DUTY CLEANER AND A GALLON OF BLEACH WERE STORED ON THE DAYCARE KITCHENETTE COUNTER NEXT TO PANCAKE SYRUP , THE CHEMICALS WERE REMOVED TO A CABINET AT THIS TIME.
Cleaning equipment shall be properly stored away from food, utensils, equipment, and clean linens. REMOVE UNUSED COOLERS FROM DAYCARE ROOMS.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. REPAIR OR REPLACE DAMAGED AND UNSANITARY KITCHEN COUNTERS.
CLEAN OUT THE DUSTY KITCHEN HOOD FILTERS. REPAIR THE OUT OF ORDER 1 COMPARTMENT SINK IN THE KITCHEN. REPAIR THE 'DRAGONFLY' ROOM TOILET HANDLE.
CLEAN OUT THE DEAD BUGS FROM THE 'FIREFLIES' ROOM LIGHT SHEILD.
The walls and ceilings shall be in good repair and easily cleaned. REMOVE THE WATER DAMAGED CEILING TILES FROM SCHOOL AGE WASHROOM AND REPLACE MISSING TILES THROUGHOUT THE SCHOOLAGE CLASSROOMS.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REPAIR THE DAMAGED AREAS BEHIND THE RADIATORS IN THE FRENCH ROOM.

THE KITCHEN WHERE DAY CARE MEALS ARE PREPARED HAS A BROKEN AND UNUSABLE EXPOSED HAND SINK, REPAIR THE EXPOSED HAND SINK SO IT IS FULLY FUNCTIONAL. THE SECOND NON-FUNCTIONAL SINK IN THE KITCHEN MUST BE REMOVED AND CAPPED OR BE FULLY FUNCTIONAL.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE MILDEWED FLOOR AND BASE BOARD UNDER THE BROKEN SINK NEEDS TO BE SANITIZED.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. PRODUCT WAS STILL BELOW 40 F AND WAS MOVED TO CONTAINER AND PLACED UNDER COLD RUNNING WATER.
