The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. CLEAN THE VENTS THROUGHOUT PREMISES HAS DUST AND DIRT BUILDUP. REPAIR OR REPLACE THE FAUCET AT THE THREE COMPARTMENT SINK IT IS SPRAYING WATER ALL OVER WHEN TURNED ON.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. REPLACE THE GASKET AROUND THE DOOR AT THE WALKIN COOLER IT IS IN POOR REPAIR. NEEDS TO MAKE A TIGHT SEAL SO NO AIR EXCAPES.

OVER 25 MOUSE DROPPINGS WERE FOUND IN THE CORNERS OF THE REAR STORE ROOM AND TWO DEAD MICE WERE FOUND IN A NEARBY CATCHALL TRAP, THREE HOLES WERE FOUND IN THE NORTH OUTER WALL BRICK WORK, REPAIR THE WALL, CLEAN UP ALL EVIDENCE OF PEST'S AND CHECK THE BUILDING PERIMETER FOR ADDITIONAL HOLES AND HARBORAGE. HAVE THE PEST CONTROL PROVIDER PUT A COPY OF THEIR CURRENT BUSINESS LICENSE IN THE LOG BOOK.
THE WALK IN COOLER AND SODA FOUNTAIN SHELVES ARE DIRTY AND DAMAGED. THE CLEAN, REPAIR, RESEAL OR REPLACE DAMAGED AND UNSANITARY SURFACES.
THE FLOORS UNDER THE COOKING EQUIPMENT AND SODA FOUNTAIN ARE BUCKLING, AND THE BASEBOARDS ARE DAMAGED THROUGHOUT THE PREMISES, REPAIR THE FLOORS AND BASEBOARDS SO THEY ARE SMOOTH AND SANITARY.
