All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED OVER 75 RAT DROPPINGS ALONG BASEBOARDS THROUGHOUT FAR REAR STORAGE ROOM AND OPEN 2 INCH GAP ON BOTTOM AND SIDE OF GARAGE EXIT DOOR OF THAT ROOM. PRIMARY DRY FOODS STORAGE ROOM FOUND ALL GARAGE EXIT DOORS WITH OPEN 1/2 AND 1 INCH GAPS ALONG BOTTOM OF DOORS. RECENT PEST CONTROL REPORT NOTED NO ACTIVITY. INSTRUCTED MUST REMOVE ALL DROPPINGS, SANITIZE ALL AREAS AND MAKE ALL EXIT DOORS TIGHT FITTING ON ALL SIDES.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. WASHROOM DOOR MUST HAVE SELF-CLOSING DEVICE.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST PROVIDE TOILET TANK TOP. MUST PROVIDE PROPER STOPPERS FOR THREE COMPARTMENT SINK AND MAKE SINK ACCESSIBLE. THREADED NOZZLE ON FRONT UTILITY MOP SINK MUST BE REMOVED OR PROVIDE BACK FLOW PREVENTION DEVICE.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. PROVIDE FOR STORAGE ROOM COOLER.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE FOR REAR DRY FOOD STORAGE.
The walls and ceilings shall be in good repair and easily cleaned. MUST REPLACE MISSING CEILING TILES THROUGHOUT THREE COMPARTMENT SINK ROOM.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. CEILING VENTILATION IN WASHROOM WITH EXCESSIVE DUST MUST CLEAN.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. ALL UNNECESSARY ARTICLES THROUGHOUT BOTH STORAGE AREAS MUST BE REMOVED AND ALL OTHERS MUST BE STORED ELEVATED AND ORGANIZED.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS THROUGHOUT PRIMARY STORAGE ROOM WITH DIRT, DEBRIS. MUST CLEAN AND ALL STORED ITEMS MUST BE ELEVATED 6' OR ABOVE.

MAKE EXIT DOOR TIGHT FITTING ON ALL SIDES.
