All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED EVIDENCE OF RODENT ACTIVITY THRU-OUT BASEMENT STORAGE ROOMS ON FLOORS AND ALONGS WALLS; OVER 15 MICE DROPPINGS INSIDE STAFF LOCKER ROOM, 30 PLUS INSIDE ELECTRICAL ROOMS, AND OVER 90 DROPPINGS INSIDE OFFICE FURNITURE STORAGE ROOM. MUST CLEAN-SANITIZE AREAS AND SEAL ANY OPENINGS TO PREVENT PEST ENTRY. ALSO CLEAN ALL DEAD ROACHES ON FLOORS IN ELECTRICAL CLOSETS. ADVICED TO CALL PEST CONTROL FOR SERVICE. OVER 30 LIVE AMERICAN COCKROACHES WERE FOUND UNDER THE FLOOR DRAIN LID IN THE KITCHEN UTENSIL/EQUIPMENT DRY STORE ROOM AND IN THE BASEMENT SUMP PUMP AREA. OVER 50 MOUSE DROPPINGS WERE FOUND IN THE GYMNASIUM STORE ROOM UNDER THE LOW SHELVING, IN THE CABINETS IN SCIENCE ROOMS # 203, 310 AND 313. DAMAGED BASEBOARDS AND HOLES IN THE CABINETS AROUND PIPE CHASES WERE FOUND. REMOVE ALL EVIDENCE OF PEST ACTIVITY AND SEAL UP THE HOLES WHERE PEST HARBORAGE IS EVIDENT.
THE GYMNASIUM STORAGE ROOM SHELVES ARE TOO LOW TO BE SWEPT AND MAINTAINED, RAISE THE SHELVING TO 6 INCHES SO THE FLOORS UNDER CAN BE SWEPT AND MAINTAINED. THE ICE MACHINE DOOR IS DIRTY; WASH, RINSE AND SANITIZE THE DOOR.
THE CEILINGS HAVE SOME SOAKED TILES IN THE 3RD AND 2ND FLOOR SERVING AREAS AND IN FRONT OF THE 1ST FLOOR TEACHERS WASHROOM. REPLACE THE DAMAGED CEILING TILES.
THE SCIENCE LAB SINK DRAINS ARE PARTIALLY CLOGGED, CLEAN OUT THE DRAINS. THE 3RD FLOOR HAS ONE OUT OF ORDER URINAL, REPAIR THE PLUMBING.
