Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.END CAPS MISSING TO LIGHT FIXTURES IN REAR ROOM.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.INTERIOR TOPS TO CHEST FREEZERS ARE IN POOR REPAIR-MUST REPAIR/REPLACE.

Found inadequate pest control approxmately 10 rodent(mice)droppings found in the dry storage room and rear door leading to alley not insect/rodent proofed 1/4' mode of entry all accross the bottom of said door also pest control log book is incomplete missing updated IDPH license map locator indicating pest control services. Serious violation 7-38-020.
Found the 3 compartment sink not maintained i.e. waste water not properly draining out,sink stoppers missing same sink.Must repair to good working condition as required.Serious violation 7-38-030.
Floors along walls and in all corners throughout in need for detail cleaning and replace broken floor tiles.
Must detail clean,sanitize walls above the 3 compartment sink and replace water stained ceiling tiles throughout.
Must remove pieces of unused kitchen equipments due to potential pest harborage 2nd warning.
