All dishwashing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health. WASTE WATER BACKING UP FROM FLOOR DRAIN FOR 3 COMPARTMENT SINK FLOWING ONTO FLOOR. MUST MAINTAIN DRAIN.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. 15 MOUSE DROPPINGS NOTED IN ROOM IN FRONT OF LIVE CHICKEN STORAGE AREA. ALSO NOTED OVER 1/4' GAPS UNDER FRONT AND REAR DOORS AND ON THE LEFT SIDE OF REAR DOOR. INSTRUCTED TO CONTACT PEST CONTROL, CLEAN AND SANITIZE EFFECTED AREAS AND CONTROL ACTIVITY. ALSO INSTRUCTED TO RODENT PROOF ALL DOORS.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. AREA WITH CRATES AND DEBRIS OUTSIDE, RIGHT OF REAR DOOR MUST BE CLEANED.

INSTRUCTED TO REMOVE ELECTRIC ZAPPER UNITS AND GLUE STICK, PROVIDE A 16 MESH TO THE INCH SCREEN TO WINDOW FANS/VENTS AND RODENT/INSECT PROOF DOORS. SERIOUS VIOLATION: 7-38-020.
