All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.found/observed front entrance/exit door not rodent proofed, door had 1/4inch gap on bottom of door, also found observed rodent droppings(20 )in front storage areas by shelving in front window area.citation issued H000055947 chapter 7-38-020 (serious violation)your court date is on 3/18/2010 at 400 w superior in room 112, at 9:00 am.
All food not stored in the original container shall be stored in properly labeled containers. The licensee shall inform the public in writing of the Chicago Department of Public Health?s Consumer Advisory relating to consuming raw and undercooked animal products.found food items not properly dated/labeled in coolers must date and label all food items in coolers.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.non food contact surfaces of conter shelving that had peeling paint shall be repair/replaced.

INSTRUCTED MANAGER THAT ALL POTENTIALLY HAZARDOUS FOODS MUST MAINTAIN A TEMPERATURE OF 40F OR BELOW AND MUST BE STORED INSIDE THE REACH IN COOLER NOT IN ROOM TEMPERATURE. MANAGER DISCARDED THE SAID PRODUCT WORTH $64.00,TOTAL 12 LBS. CITATION ISSUED CRITICAL.

FOUND FROZEN FOOD FROM UNAPPROVED SOURCE. FOOD WAS STORED INSIDE THE FREEZER IN SELLING AREA. EACH ZIPLOC BAG HAD PIECES HOME MADE 'CHICKEN SAMOSA',TOTAL BAGS OF 23; 6 PIECES EACH ZIPLOC BAG OF HOME MADE' BEEF SHAMI KABOB', TOTAL BAGS OF 14; 10 PIECES EACH OF 'CHICKEN EGG ROLL',TOTAL BAG OF 1. ALSO FOUND TWO CONTAINERS OF 1POUND EACH OF 'BARKAY SWEETS' ON THE LABEL OF THIS TWO ITEMS CLEARLY STATES home made. MR.AKATAR ALIMEGANI STATED THAT FOOD IS MADE AT HOME OF CUSTOMERS. FOOD WAS DISCARDED AND DENATURED, ALSO INSTRUCTED MANAGER ALWAYS BUY FOOD FROM A RETAIL FOOD ESTABLISHMENT AND KEEP INVOICES/RECEIPTS ON PREMISES.POUNDS 40, VALUE 311. CRITICAL VIOLATION:7-38-005(B)
