All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST PROVIDE LIDS FOR BAR ICE HOLDING UNITS. MUST NOT USE PAPER AS LINER FOR SHELVING AT REAR BAR. MUST INSTALL SPLASH GUARD BETWEEN KITCHEN EXPOSED AND ONE COMPARTMENT SINKS.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. BAR ICE SCOOP MUST BE STORED IN A CLEAN CONTAINER. NOT ON TOP OF MACHINE. EXPOSED TABLEWARES MUST BE COVERED/WRAPPED. ALL TO GO TIN CONTAINERS AND POTS/PANS MUST BE STORED INVERTED.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. ALL STORED ITEMS IN FRONT BAR CLOSET AND REAR BAR MUST BE ELEVATED FROM THE FLOOR 6' OR ABOVE.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. THREADED NOZZLES ON REAR BAR TWO COMPARTMENT AND KITCHEN PREP ONE COMPARTMENT SINKS MUST BE REMOVED. MUST INSTALL EXPOSED HAND SINK IN REAR PREP ROOM WHERE FLOOR DOUGH MIXER IS USED OR REMOVE MIXER AND ALL OTHER FOOD PREP EQUIPMENT.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE A LIGHT SHIELD OR SHATTER PROOF BULB AT DISH MACHINE AND CLEAN DISH STORAGE AREA.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. BASEMENT THROUGHOUT MUST BE CLEANED, ORGANIZED. ALL UNNECESSARY ITEMS REMOVED AND ALL OTHER ARTICLES STORED ELEVATED AND ORGANIZED. REMOVE ALL POOLING WATER.

OBSERVED THE FOLLOWING FOODS AT IMPROPER TEMPERATURES: SAUSAGE AT 43.7F, MEAT RAVIOLI AT 51.2F, CHICKEN AT 49.2F, WHIPPING CREAM AT 43.7F, MOZZARELLA CHEESE AT 43.8F, COOKED PASTA AT 48.7F. MANAGEMENT VOLUNTARILY DISCARDED 20# OF FOOD WORTH $100. CRITICAL VIOLATION 7-38-005A.
FOOD (ICE) NOT PROTECTED FROM CONTAMINATION DURING STORAGE INSIDE OF THE ICE MACHINE. OBSERVED MOLD GROWTH ON INTERIOR CONTACT SURFACES OF THE ICE MACHINE. MUST TURN OFF MACHINE, REMOVE ICE AND HAVE INTERIOR SURFACES CLEANED AND SANITIZED. UNIT TAGGED HELD FOR INSPECTION. DO NOT USE MACHINE UNTIL HELD FOR INSPECTION TAG HAS BEEN REMOVED. SERIOUS VIOLATION 7-38-005A.
OBSERVED LIVE HOUSE FLIES ON THE PREMISES. 10 HOUSE FLIES WERE OBSERVED IN THE BASEMENT. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
NO CERTIFIED FOOD MANAGER WAS PRESENT WHILE POTENTIALLY HAZARDOUS FOODS (PIZZA AND PASTA) WAS BEING PREPARED AND SERVED. THE CERTIFIED FOOD MANAGER ARRIVED ONSITE AT APPROXIMATELY 3:15PM. MANAGEMENT INSTRUCTED THAT A CERTIFIED FOOD MANAGER MUST BE PRESENT AT ALL TIMES WHILE POTENTIALLY HAZARDOUS FOOD IS BEING PREPARED OR SERVED. SERIOUS VIOLATION 7-38-012.
PREVIOUS MINOR VIOLATIONS FROM REPORT #517381 ON 6/1/11 NOT CORRECTED: 32/01 - MUST INSTALL SPLASH GUARD BETWEEN KITCHEN EXPOSED AND ONE COMPARTMENT SINKS. 36/01 - MUST PROVIDE A LIGHT SHIELD OR SHATTER PROOF BULB AT DISHMACHINE AND CLEAN DISH STORAGE AREA. 41/01 - BASEMENT THROUGHOUT MUST BE CLEANED, ORGANIZED. UNIT WAS USED TO STORE POTENTIALLY HAZARDOUS FOODS SUCH AS CHICKEN, COOKED PASTA, CHEESE, ETC. AIR TEMPERATURE WAS AT 42.9F AT THE END OF THE INSPECTION. MUST MAINTAIN COOLER BELOW 40F. CRITICAL VIOLATION 7-38-005A.
