The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DETAIL CLEAN THE FLOOR AROUND THE 3 COMPARTMENT SINK AND THE GREASE TRAP.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. REPAIR THE LEAKING FAUCET AT THE 3 COMPARTMENT SINK.

OBSERVED BEVERAGE COOLER AT IMPROPER TEMPERATURE OF 52F. COOLER WAS USED TO STORE POTENTIALLY HAZARDOUS FOODS SUCH AS MILK, HALF AND HALF AND ORANGE JUICE. UNIT WAS TAGGED HELD FOR INSPECTION. MUST MAINTAIN THE COOLER BELOW 40F. FAX A REINSPECTION LETTER TO (312) 746-4240 TO HAVE THE HELD FOR INSPECTION TAG REMOVED. CRITICAL VIOLATION 7-38-005A.
