All employees shall be required to use effective hair restraints to confine hair. COOKS MUST WEAR HAT/HAIR RESTRAINTS.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. BROKEN INTERNAL THERMOMETERS IN TALL REACH-IN FREEZER AND COOKS LINE COOLER. MUST REPLACE.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. ICE SCOOPS MUST BE STORED IN A CLEAN CONTAINER WITH A SANITIZING SOLUTION. NOT ON TOP OF ICE MACHINE.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. COOKS LINE TALL REACH-IN COOLER DOOR HANDLE BROKEN AND REPAIRED WITH DUCT TAPE. MUST REPAIR/REPLACE WITH CLEANABLE MATERIAL.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOOR UNDER SLICER PREP TABLE, BEHIND ICE MACHINE WITH FOOD DEBRIS. MUST CLEAN. MUST PROVIDE FLOOR DRAIN COVER AT ICE MACHINE.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE LIGHT SHIELD ABOVE ICE MACHINE.
All food shall be protected from contamination and the elements, and so shall all food equipment, containers, utensils, food contact surfaces and devices, and vehicles. OBSERVED THE FOLLOWING CROSS CONTAMINATION. INTERIOR OF ICE MACHINE IN BOTH UPPER WATER RESERVOIRS WITH BLACK AND PINK SLIME SUBSTANCE DRIPPING DOWN INTO ICE. ALL ICE DISCARDED. MACHINE TAGGED HELD FOR INSPECTION AND MUST NOT USE UNTIL CLEANED, SANITIZED AND FAX REQUEST FOR RE-INSPECTION AND TAG REMOVAL OF ICE MACHINE TO 312-746-4240. OBSERVED APPROXIMATELY 5LBS COOKED PASTA IN A STRAINER IN THE KITCHEN MIDDLE COMPARTMENT OF THE THREE COMPARTMENT SINK WHILE THE OTHER WASH AND SANITIZE COMPARTMENTS WERE SET. DIRTY PANS OBSERVED IN THE WASH COMPARTMENT AND A WET, DIRTY TOWEL HANGING OVER SINK NOZZLE ABOVE THE CONTAINER OF PASTA. PASTA DISCARDED.
One copy of the Food Inspection Report Summary must be displayed and visible to all customers. SUMMARY REPORT FROM 6-24-11 NOT POSTED VISIBLE TO CUSTOMERS. SUMMARY WAS COVERED BY SEVERAL OTHER SIGNS AND UNABLE TO SEE.
All food not stored in the original container shall be stored in properly labeled containers. ALL PREPARE FOODS IN WALK-IN COOLER MUST BE LABELED AND DATED.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. TABLETOP CAN OPENER FOUND RUSTY WITH ENCRUSTED FOODS. MUST CLEAN AFTER EACH USE.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. NOZZLES ON KITCHEN THREE COMPARTMENT SINK AND MIDDLE BAR THREE COMPARTMENT SINK WITH LEAKS. MUST REPAIR. THREADED NOZZLES ON FRONT BAR AND PANTRY EXPOSED HAND SINKS. MUST REMOVE.
