All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Interior panels of ice machines at 1si & 2nd floors need cleaning, including the fan guards of coolers & some bar sinks, interior of one fryer- due to debris dust or stains.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Floors around equipment in the bars, under a wait staff room by first floor bathrooms, under stairs beyond the kitchen used for dry storage area need cleaning & all stock/items must be elevated off the floor.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Must fix or replace a leaky faucet at 3-comp sink across the dishmachine.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. Any old equipment, clutter or unnecessary articles in various storage rooms at both floors must be removed or better organized, to eliminate pest harborage areas.

A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. The kitchen staff is cooking and preparing meats and stuffed meat items and the certified manager did not arrive in the restaurant until 45 minutes had passed.
A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. Per report # 277095 dated July 6, 2010 the following violations were found again and include, The interior panels and sender arm of the 1st floor ice machine have an slimey unsanitary buildup which needs to be removed from the machine or the unsanitary parts replaced to reduce possible contamination.

THE MAIN DISHMACHINE, 1ST FLOOR BAR AND 2ND FLOOR BAR DISHMACHINES HAVE NO SANITIZING RINSE, ALLTHREE DISHMACHINES ARE TAGGED HELD FOR INSPECTION UNTIL ADEQUATE SANITIZER HAS BEEN RESTORED TO THE MACHINES AND ARE REINSPECTED BY THE CHICAGO DEPARTMENT OF PUBLIC HEALTH. 0 PPM CHLORINE DETECTED IN ALL 3 CHLORINE BASED SANITIZING MACHINES.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments. OVER 30 LIVE SMALLFLIES WERE FOUND IN THE BARS AND DISH/MOP AREA. TWO LIVE COCKROACHES WERE FOUND ON THE KITCHEN FLOOR AND NUMBEROUS DEAD ROACHES WERE UNDER THE HOT WATER HEATERS. REMOVE ALL EVIDENCE OF ANY PEST ACTIVITY.
