Floor not properly sealed in prep area. Instructed to apply sealant to surface and under equipment.
Raw wood around front counter area and rear storage shelves not sealed. Instructed to apply non-toxic sealant to surface.
Outer openings not protected or rodent proofed. Rear exit door has 1/2 inch gap across entire bottom of door. Instructed to rodent proof door to prevent rodent and insect entry. No citation issued, establishment not opened.
No hot running water under city pressure provide. Establishment must provide hot water under city pressure during all hours of operation. No citation issued, establishment not open.
Facilities not maintaining proper temperature. Instructed to have all coolers on and running at 40 F or below. No citation issued, establishment not open.

The walls and ceilings shall be in good repair and easily cleaned. PEELING PAINT ON WALLS ABOVE PREP AREA MUST BE REMOVED,REPAINT WITH NON TOXIC
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. EXTERIOR OF ICE MACHINE NOT CLEAN
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. ICE SCOOP MUST BE STORED IN A SANITIZED CONTAINER

Found food items ot properly dated/labeled in coolers, reach in freezer, must date and label food items in coolers, reach in freezer.
Cooler shelving/racks that had peeling paint/rust, shall be replaced/repaired. Storage shelving that had exposed raw wood shall be repaired/sealed.
Non food contact surfaces of cooler shelving, prep table lower shelving not clean, need detailed cleaning(crevices).
Floors under rear storage area, and furnace area not clean need detailed cleaning(corners).
Missing light shields in cooler shall be provided, light shields in prep area need cleaning.
Exhaust vents(ventilation) in coolers, bathrooms not clean, need detailed cleaning.
