--Some of the racking in the walk in deli cooler shows the chrome plating peeling, missing, etc and shows obvious areas of corrosion. The east wall behind the utensil shelving area in the salad bar prep room shows paint chipped and flaking over and adjacent to areas where the cleaned and sanitized utensils are stored.

--One stem thermometer in deli/hot foods indicated a +4*F (36*F) in ice water bath.

--Dried food product,spilled material (food and non food) observed on the west wall in the grocery stock room under Pasta and sauce storage rack. Dried food product build up observed on the interior frame of the tenderizer/packing station and in the interior of the Hobart Filmizer. Both proofing boxes in the bakery show spilled product and what appears to be black mold on the floor perimeters. Some old product on the bottoms of the blue steel shelving in the seafood walk in cooler.

--In various areas of bakery and deli cleaned and sanitized utensils (scoops, spatula., songs, etc.) are being stored in white plastic containers (approximately the size of a 5 lb salad container) that shows dried food reside in the bottom. There is a large round mirror Used for wedding cakes) in the bakery that has cracks in the glass and is stored on top bulk ingredient containers behind the Oliver slicer in the bakery.
- Air Gap Requirement, Design of Backflow Prevention & Conditioning Devices (46.822(c)-(e))
--The black hose in the meat room resting in the base of the rinse section of the three bay sink has a hose bib vacuum breaker attached by the hose has a pistol grip valve attached which defeats the purpose of the vacuum breaker at the bib.

--One sagging ceiling tile in the bakery over the three bay sink.
- Drying Mops (46.981(f))
--Wet mops observed in various areas including bakery, adjacent to slop sink and in meat prep room which may result in area subject to sour fly like insects.
