- KDA_3_30212 - --Under the wooden cutting board, there were three containers with white powder in them with out a common name. Person in charge said they were, sugar, salt and flour. On a shelf near the wooden cutting board thee was a clear shaker with a yellow lid and a clear container with a green lid with out a common name. Person in charge said they were curry and garlic. COS-containers were labeled.
- KDA_3_50118A1 - --In the walk in cooler, there was a container of tumbler(cheese, cooked bacon and cooked on site chicken mixture) that was date marked 9/14/15. Mixture was held over seven days. COS-person in charge discarded the mix.
- KDA_3_50118A2 - --In the walk in cooler, there were single serve made on site cooked pasta with out a date. Person in charge said they were cooked yesterday. COS-pasta was dated with 9/20/15. This is less than 10% of the food they have with date marks. ( FWL) was issued.
- KDA_4_20211A2 - --On a shelf in the wear washing area, there were five plastic containers that had cracks along the food contact surface. COS-Person in charge discarded the containers.
- KDA_4_501114C2 - --The spray bottle of sanitizer was at 150PPM quat. COS- Employee made a bucket of sanitizer to 200PPM.
- KDA_4_60111A - --At the waitress station, a pizza cutter was stored as clean with dried on food debris. COS-pizza cutter was removed to be rewashed. In the back hallway, the lid that is on the wheat flour, has an accumulation of grease and dirt.
- KDA_4_60111C - --The shelf below the make table that stores the pizza boxes, has an accumulation of food debris. COS-Shelf was cleaned.
- KDA_4_60211E4 - --The metal ledge in the ice machine has black mold on it. Person in charge said they will add ice machine to weekly cleaning list.
- KDA_4_90311A - --In the back hallway, there was a box of 4oz. Styrofoam containers sitting on the floor. COS-person in charge moved them off the floor.
- KDA_6_50111 - --The base board near the ice machine has a large hole in it.
- KDA_6_50112A - --The light switch in the employee restroom has accumulation of dirt. The walls through the kitchen have an accumulation of dirt. The vent covers have an accumulation of dust.
- KDA_Defacto - --Frozen food is frozen. --In the make table: cooked sausage 43F, reach in make table, cooked on site diced chicken 42.3F. Ambient of Aquafina reach in 40.5F. Walk in cooler: cooked ground beef 39.3F. --marinara sauce in soup pot 142.6F Educated on proper heating within two hours and hot holding at 135F.
