Employee said, he just sat it there before the lunch rush (30 min ago). COS-chili was placed in walk in cooler. KDA_3_50116A1 - --On the steam table, refried beans 110.2F. Employee said they had been hot holding for two hours. COS-beans reheated to 165F. KDA_3_50118A2 - --In the walk in cooler, a pan of made on site rice was with out a date mark. Employee said it was made two days ago-12/8/15. COS-rice was dated. Less then 10%-so issued a FWL. KDA_4_20211A2 - --At the end of the three compartment sink, a rubber spatula and one food container had cracks along the food contact surface. COS-items were discarded. KDA_4_20311 - --The cooking thermometer they had available for taking food temperatures was not working correctly. COS-they had two others available. Educated on calibration. KDA_4_60111C - --The shelf located below the make table had dust and debris on it. KDA_4_90311A - --Sitting beside the pop concentrates, a box of 200 single serve Styrofoam containers were sitting on the floor. Person in charge said they are defective, and need to be discarded. In the cash register area, a box of 32 oz, 500 count single serve cups were sitting on the floor. COS-items were removed from the floor. KDA_4_90311B - --The ice bucket and ice scoop was sitting face up on top of the ice machine. COS-items were placed for rewashing and will be placed on a shelf next to the ice machine and covered with a towel. KDA_6_20214 - --The men's and women's restroom doors do not have self closures. Person in charge said he just removed them when they had flooring dug up and plumbing repaired in kitchen. He will get self closures on them. KDA_6_30114 - --There is no hand washing signs in the men's and women's restroom. KDA_6_50111 - --There is wall tile missing on the corner wall, when you enter the kitchen area. The shelf in the walk in cooler is broken and leaning to the right. KDA_6_50112A - --There is dried on food debris above and below the three compartment sink. The fan guards in the walk in cooler have an accumulation of black debris. The wall under the drive thru window has dried on food debris. There is is dried on debris on the wall by the pop containers in the storage area. Under the shelf in the storage area, there is a large clump of dirt. Person in charge said he was unaware that it was there. He said that it looks like debris from when they had floor dug up and plumbing repaired. KDA_6_50119 - --The men's and women's restroom doors were open during business hours. Person in charge said, they had floors in kitchen dug up and plumbing replaced, so doors have been left open to help with odor control. COS- doors were closed. KDA_7_20111B - --In the storage area, located on the top shelf the following items were stored above single serve cups and individually wrapped tooth picks: Pro Mar 200, gallon of green paint, polycrylic and high gloss lacquer. COS-chemicals were removed and placed on bottom shelf. KDA_Defacto - --Frozen food is frozen. --On the make table sitting in ice, shredded cheese 39.9F. walk in cooler, made on site rice 41.7F. customer self serve area, volcanic sauce 41.2F. --On the steam table, taco meat 179.2F. --Reheated refried beans in the oven to 165F.
