COS- labeled. Located on a shelf next to MT 1, there was an unlabeled container of black and white powder and an unlabeled container of green substance which the PIC identified as salt and pepper and parsley, respectively. COS- labeled. Located on a custard prep table, there was an unlabeled container of pink powder which the PIC identified as crushed peppermint. COS- labeled. Located on a shelf next to the mop sink, there was an unlabeled container of clear liquid which the PIC identified as water. COS- labeled.
- KDA_3_30412B - --Located in the back storage area, there was a plastic container with no handle used as a scoop stored inside the container of sea salt in contact with the food. COS- removed.
- KDA_3_30511A1 - --Located in the walk in freezer, there was a box of strawberry halves stored on the floor.
- KDA_3_50118A2 - --Located in the Kenmore RIC, there was an opened package of hot dogs with no date mark. The PIC said that they had been opened on 1/2. COS- dated. Located in the RIC under the icecream topping MT, there was a container of cheesecake that had no date. The PIC said that the cheesecake had been pulled from the freezer on 12/31. COS- dated, educated that the unused cheesecake will need to be discarded at the end of the day (today is the 7th day).
- KDA_4_60111A - --Located on the dish shelf in the back kitchen area, there were three large plastic food containers, four large plastic food container lids, and two small crates of approximately 40 plastic food container lids each that were soiled with stickers. COS- moved to wash area. Located on the dish shelf in the back of the kitchen, there was one plastic food container stored as clean that was soiled with dried food debris. COS- moved to wash area.
- KDA_4_60211E4 - --There is mold on the inside rim of the kitchen ice machine.
- KDA_4_70211 - --There was no detectable sanitizer in the warewash machine after a tray of dishes had been washed in it. The water temperature of the warewash machine was 129.4F at plate level. COS- The person in charge (PIC) changed the sanitizer container as the one that they were using had a hose that did not reach all the way to the bottom of the container and there was not much sanitizer left in the container. The chlorine sanitizer was at 100ppm after the containers were changed. Dishes were rewashed.
- KDA_4_90311A - --Located in the back storage room, there was a box of single use foam cups stored on the floor.
- KDA_6_30111 - --There was no soap available at the front kitchen hand sink. Corrected on site (COS)- provided soap.
- KDA_6_50112A - --The wall under the warewash machine is soiled with debris. The vent cover on the ceiling in the back kitchen is soiled with dust.
