KDA_3_30212 - --Located at the back of the kitchen there were three unlabeled containers of white powder which the PIC identified as dry milk, flour, and wheat flour. COS- labeled. Located on a shelf above the back prep table, there was an unlabeled food container of brown powder which the PIC identified as cinnamon sugar. COS- labeled. KDA_3_30412B - --Located at the back of the kitchen, there was a bin of wheat flour with the scoop stored inside with the handle of the scoop in contact with the flour. COS- moved handle above food. Located on a shelf above the back prep table, there was a food container of cinnamon sugar with a single use foam cup with no handle stored inside to be used as a scoop. COS- removed. KDA_4_20211A2 - --Located in a drawer at the back of the kitchen, there were two rubber spatulas that were chipped along the edges. COS- removed from drawer to have fixed. Located at the dish shelf by the three vat sink, there were two plastic food container lids that were cracked and chipped. COS- discarded. Located in a drawer under the prep table, there were six rubber spatulas that were chipped around the edges. COS- removed from drawer to be fixed. KDA_4_60111A - --Located on the dish shelves at the back of the kitchen, there were two metal food containers stored as clean that were soiled with dried food debris. Corrected on site (COS)- moved to wash area. Located on the dish shelves at the back of the kitchen, there were three metal food container lids and ten plastic food container lids stored as clean that were soiled with sticker residue. COS- moved to wash area. KDA_4_60111C - --The fan guards of the walk in cooler and walk in freezer are dusty. KDA_4_70211 - --The chlorine warewash machine had no detectable chlorine sanitizer. There had been a few trays of dishes that had been run through the machine. The pump on from the container of sanitizer to the machine did not seem to be working. The maximum temperature of the unit was 151.2F. COS- the dishes that had been run through were rewashed in the three vat sink and the person in charge (PIC) said that they would use the three vat until the dishwasher was fixed. KDA_4_90311B - --Located on the dish shelf, there was a stack of five metal food containers stored as clean that were stacked wet. KDA_6_50111 - --The clean dish shelves at the back of the kitchen have chipping paint. KDA_7_10211 - --Located on the chemical shelf under the prep sink, there was an unlabeled spray bottle of blue liquid which an employee identified as soap and water. COS- labeled. KDA_7_20111A - --Located on the shelf over the prep table, there was a plastic food container that had a bottle of hand sanitizer stored next to containers of spices, honey, and hot sauce. COS- moved hand sanitizer.
