KDA_3_30212 - --There are several squeeze bottles in the make table and around the grill that are not labeled. Sitting on a shelf next to the glass front reach in, two metal shakers were with out labels. One had a black and brown powder in it, the other had a yellow/white powder in it. Employee said they were salt for the fries and the other was seasoning for the chicken. Employee said they label them with black market and it doesn't stay. Employee said they will get labels. KDA_3_30414B1 - --There was a moist wiping cloth sitting on a red bull cooler in the bar. KDA_3_50116A2 - --Sitting in a pan of ice to the left of the make table, shredded lettuce 49.7F. Employee said lettuce had been out since 11:00 AM. COS-lettuce was placed in walk in cooler. KDA_3_50118A1 - --In the walk in cooler, a pan of made on site vegetable beef soup was held over seven days. The date marking on it was 12/14- 12/20. In the single door glass reach in, cooked on site chopped chicken was held for over seven days. The date on the chicken was 12/14-12/20. COS- items were voluntarily discarded. KDA_3_50118A2 - --In the single door glass reach in, single serve bowls of BBQ beef were with out date marking. Employee said it was make on Saturday 12/19/15. COS-beef was dated correctly. Since there was only one item, Field warning letter was issued. KDA_4_20211A2 - --Stored with utensils under the make table, a rubber spatula had cracks in it. A metal spatula handle was taped with black tape. One pair of tongs had a melted end on the food contact surface. COS-these items were removed from the area. KDA_4_60111A - --Under the make table hot holding gravy, there is a dicer that has dried on food debris. Stored as clean on a shelf in the ware washing area, one lid had sticker residue and one lid had dried on food debris. COS- items were placed at three compartment sink. KDA_4_60111C - --The vent hood above the fryers, have an accumulation of dried on grease and fresh grease. The fans in the walk in cooler have an accumulation of dust. The vent cover on the single door glass reach in, have an accumulation of dust. KDA_4_60211E4 - --The plastic ice deflector in the ice machine, has brown and red mold on it. The area's around the pop nozzle have a build up of debris. KDA_4_90311A - --In the back storage area, there was a case of miller single serve cups that were sitting on the floor. COS-cups were moved off the floor. KDA_4_90311B - --Sitting on a shelf under the make table, six metal pans were stacked wet. COS-pans were placed at the three compartment sink. KDA_5_50117 - --In the unisex restroom in the party room, there is not a covered trash can. KDA_6_20214 - --In the party room, the unisex restroom door is not self closing. KDA_6_20215A3 - --There is about a 1/2" gap in the bottom of the back door. The front door, has about a 1" along the bottom. The interior glass door, has about a 1/2" gap along the bottom. KDA_6_30114 - --In the men's restroom and the unisex restroom in the party room, there is no hand washing signs. COS(corrected on site)-signs were left. KDA_6_50111 - --In the kitchen area, there are several loose, cracked and stained ceiling tile. Underneath the soap dispenser at the hand washing sink in the kitchen, the sheet rock is tore. In the women's rest room the floor trim is loose. KDA_Defacto - --Frozen food is frozen. --On the grill, chicken 165.2F. --On the make table right of the kitchen entry, white gravy 137.9F. On the steam table in front of the grill, refried beans 135.7F. Sitting on the back of the grill, shredded beef 138.7F. --On the top of the make table, diced tomatoes 41.6F. Sitting in a pan of ice to left of the make table, shredded lettluce 43F. In the clear front reach in, individual bowl of BBQ beef 33.5F.
