KDA_3_30412B - --In ice machine, handle to ice scoop left in ice. COS removed. KDA_3_30414B1 - --Wet sanitizing rags are left out on counters and then placed in big buckets by handsink approximately every 30 minutes. Wet wiping cloths are required to be in the sanitizing buckets at all times. KDA_3_50119B2 - --Sandwich cold held items on prep line (sliced tomatoes, corn salsa, shredded lettuce, etc) are on time as control, per PIC. No time stamps on anything on sandwich cold prep line. PIC stated they all went in the line 30 minutes ago. COS labeled with time stamp. KDA_4_20211A2 - --Up front at registers, plastic cover that covers RTE cherries for ice cream is cracked. Container holding RTE lettuce in reach in cooler for salads was cracked on the corner of the container. One container stored as clean on shelving for clean equipment was cracked on the corner. COS discarded. KDA_4_204112B - --The ambient air thermometer to the reach in cooler holding bagged milk on coffee bar area is broken. KDA_4_50111B - --On breakfast prep side of kitchen, reach in cooler door does not seal properly. PIC was unsure if it was door on the hinges or the seal to the door. KDA_4_60111A - --Dr Pepper and Coke soda nozzles on both soda fountains in consumer are with severe mold buildup. COS washed/sanitized Stored as clean on shelving by 3 vat sink for clean equipment, 7 plastic containers with visible food debris on surface. COS rewashed. KDA_4_60213 - --On cookline, seals and interior of reach in freezer is soiled with food debris buildup. Flurry machine and surrounding machinery is also soiled with food debris buildup. PIC stated he has a contracted pest control company in place and he has told them about the problem.
