  • KDA_3_10111 - --There were eight packages of cryovac tilapia being stored in the glass RIC located in the front of the store with no slit in the packages. This could cause Clostridium Botulinum toxin.PIC said they were pulled less than twenty four hours. COS All packages were slit open.
  • KDA_3_30211A1a - --There was a carton of raw shell eggs being stored on a shelf directly above a package of RTE carrots located in the RIC-2. COS raw shell eggs were moved to the bottom shelf.(FWL- no leakage)
  • KDA_4_60111A - --There were two plastic container stored as clean on a shelf in the main kitchen area that had dried food debris on them form a previous use. COS items were placed in the three vat sink for cleaning. There was a commercial potato dicer mounted on the wall next to the three vat sink that had dried food debris on the blades from a previous use. The employee said it was used last night. COS Item was cleaned.
  • KDA_6_30112 - --There were no paper towels at the hand sink located across from the three vat sink. COS paper towels were placed at the hand sink.
  • KDA_6_50111 - --There are floor tiles chip and missing in the kitchen area.
  • KDA_7_20212A2 - --There were two cans of Raid ant and roach spray EPA# Reg No. 4822596 being stoed in the mop sink room. PIC said they have used in the building by the back door. COS PIC marked the cans for outside use only. PIC said they have a pest control company. NO paperwork was provided. No activity was seen by the inspector.

  • KDA_3_30212 - --Sitting on a table in the kitchen, there is a clear container with a white powder in it. The container does not have a common name on it. Person in charge said it was flour and container was labeled.
  • KDA_4_60111C - --The floor of the true reach in case has debris on it. COS-Person in charge cleaned the floor of the reach in.
  • KDA_5_20515B - --At the three compartment sink, the connection at the faucet is leaking.
  • KDA_6_30111 - --The hand sink in the kitchen, does not have hand soap. Person in charge said he will get hand soap before they open for business.
  • KDA_6_30112 - --The hand sink in the kitchen, does not have hand drying towels. Person in charge said he will get paper towels before they open for business.
  • KDA_6_50111 - --In the customer area, there are four stained ceiling tile. In the ware washing area, there are three stained ceiling tile. Person in charge said he knows they need to be replaced. There is also missing and cracked floor tile in the ware washing area. In the women's restroom there are missing tile around the trim and base board. In the men's restroom, there is a piece of cardboard covering a plumbing pipe.
  • KDA_6_50112A - --Above the coke reach in, there is an accumulation of dust on the light cover. On the wall beside the whirlpool reach in, there is an accumulation of dirt and debris.
  • KDA_Defacto - --Establishment has not received license. --Person in charge has done a lot of cleaning since inspector was here for planning inspection. He plans on doing more cleaning before establishment is opened for business. --whirl pool reach in 33.1F, freezer 3.4. Coke reach in 35.1F, McCray case 29.5F, True case 32.6F, True stainless steel reach in 38.3F. Food was frozen in the True freezer.
