- KDA_3_50116A2 - --On counter next to grill, pork was being stored at 60.6F and beef at 61.0F. PIC said items were cooked approximately two hours earlier. On counter next to grill, beef was at 54.9F. PIC said beef was cooked approximately three hours ago. In MT top, diced tomatoes were stored at 53.0F, sour cream at 55.5F, and cut lettuce at 52.6F. PIC said items were cut less than four hours earlier. In MT bottom, mozzarella was stored at 53.9F and pork at 45.2F. On counter, salsa was at 47.4F. COS: PIC chose to discard all items rather than cool or reheat.
- KDA_3_50118A2 - --In steam table, refried beans were not date marked. PIC said beans were made Monday 1/25/16. COS: PIC chose to discard beans rather than date mark.
- KDA_4_20211A2 - --The lid on a container of beef was melted and broken. COS: lid discarded.
- KDA_4_204112B - --There is no thermometer in MT.
- KDA_4_30111 - --MT was not working at time of inspection. Ambient air was 54.1F. PIC said MT stopped working approximately three hours earlier and repair person was called.
- KDA_4_30212A - --Food thermometer was still in package and not being used.
- KDA_5_30212B - --There is no cover on the clean water supply tank.
- KDA_6_30111 - --Establishment had no soap at hand sink. Corrected on site (COS): Used dish soap as hand soap. Owner decided to voluntarily close in order to purchase hand soap.
- KDA_6_30112 - --No hand drying agent was available at hand sink. COS: Used napkins as hand drying agent. Owner decided to voluntarily close in order to purchase paper towels.
- KDA_6_50112A - --There is grease buildup on the walls, especially behind the grill.
- KDA_7_10211 - --In chemical storage, a bottle of unknown liquid was not labeled. COS: PIC identified liquid as degreaser and labeled bottle as such.
- KDA_7_20111A - --A bottle of degreaser was in direct contact with the microwave. COS: bottle moved to chemical area.
- KDA_Defacto - --Bleach is the only sanitizer available in establishment
