KDA_3_30211A1b - --In the customer reach in cold case, the raw shell eggs did not have a separation between the packaged deli meat. COS-Person in charge found some clear plastic shelving and made dividers between raw eggs and deli meat. Since the raw shell eggs were not leaking and it was only one item inspector did not issue an AR. KDA_3_50118A3 - --In the walk in cooler, there were the following sausages with a use by date mark of 11/17, jalapeno, beef, and cheddar. Polish sausage in the walk in cooler had a use by date of 11/18. All of these items were date marked for more then seven days. Person in charge said that she opened them today. COS-items were dated with correct use by date. Since Person in charge knew the open date and could tell me after education about date marking, inspector did not issue an AR. KDA_4_60111A - --In the walk in cooler, the lids that were on the sausages had sticker residue on them. COS-lids were changed. KDA_6_50111 - --There is cracked tile in front of the front of the frozen food reach in. There is one stained tile in the retail area. KDA_6_50112A - --In the pop storage room, there is dirt and debris under the shelves. In the office there is dirt on the floor. KDA_Defacto - --Hot water at the hand washing sink was 121F. Person in charge said they ended up replacing the hot water heater. --The open case in the middle of the store, center left side ambient 30.7F, center right side ambient 35.2F, left side 37.8F and the right side ambient 36.1F.	11/10/2015	6	Follow-up	In
3-302.11(A)(2) - --In the reach in cooler in the soda area, the raw pork bacon and raw shell eggs did not have a separation. COS-cardboard was placed between products. KDA_3_30211A1a - --In the reach in cooler in the soda area, the raw shell eggs did not have a separation between them and ready to eat lunch meat. COS-Cardboard was placed between products. KDA_3_50115B - --In the center reach in, the lunchables that were extended out an inch or two were not cooling properly. Starting on the third row from the front the lunchable were at 41F or below. COS-repairman was here, and cleaned and checked the center unit. At 12:15 the unit was cooling at 35F. KDA_3_50116A2 - --West side of the open case in the middle of the store, Ambient of the west side 38.3F. The east side, turkey lunchables 43.2F, 43.8F, pepperoni 50.4F, ham 47.1F, 48.1F, 48.2F, 46.1F. Those temperatures came from the first two rows. The third row and back were reading 40F or below. Ambient of the east side top shelf 40.3F. We discarded a total of 15 lunchables, seven were out of temperature and the rest were checking temperatures. COs-repairman was out and cleaned and checked unit. It was cooling at 35F at 12:15. KDA_3_50118A3 - --In the walk in cooler, there were three opened packages of hotdogs that were dated 11/2. These items were dated for 8days. Person in charge said they were opened today and not counting today as day one. COS-items were dated with proper discard date. KDA_4_204112B - --The thermometer in the center reach in was not working. It was reading 50F. Person in charge said it was broken. COS-new thermometer was placed in unit. KDA_5_20212A - --The three compartment sink's hot water was 83.9F, hand washing sink in soda area was 80.7F, hand washing sink in the women's restroom was 92.3F. KDA_6_50111 - --There is one cracked floor tile in front of the frozen food. There are two stained ceiling tile in the customer area. KDA_6_50112A - --There is an accumulation of dirt and debris on the left side of the ice machine and the walk in cooler under the shelves. In the pop room, there is an accumulation of dirt and debris under the shelves. In the office there is an accumulation of dirt on the floor. The hand washing sink in the soda area has an accumulation of dirt and grease on the faucet. KDA_7_30111A - --On the retail shelf, motor oil was stored beside retail bags of sugar without a separation. COS- sugar was moved so there was a separation. KDA_Defacto - --Creamer machine, 41.5F. Packaged lunch meat 38.7F. Reach in cooler, milk 39.5F. Creamer machine, cream 41.5F. Walk in cooler, uncooked hot dogs 35.3F. Center open cooler chicken salad croissant 40.6F. Ambient of west side 38.3F, ambient of east side 39.4F. --Frozen food is frozen. --On the steam roller, hot dog 180.2F.
