Mold was on tomatoes that were in the walk in cooler. COS, food item discarded. Drain flies were inside of a litter of gin, and whiskey that were behind the bar. COS, bottles of liquor were discarded. The cooked molded pork did not have a preparation date on the container. COS, cooked pork discarded. PIC stated the tomatoes and lettuce were prepared at 9 AM this morning. COS, tomatoes ,and lettuce were placed into a ice tray for cold holding. KDA_3_50118A2 - --A an opened container of milk was in the reach in cooler without a opened package date. PIC stated the milk was opened on 1/15/15 in the morning 9AM, held more than 24 hours without date marking. KDA_4_10111A - --Flour was stored in non-food grade (Sterilite) container. Cilantro was stored in a non-food grade (Sterilite) container. KDA_4_20211A2 - --Plastic lids were stored on top of rice. The lids on the rice had cracks in the plastic. Taco shells were stored in a black food storage bins with holes in the plastic. KDA_4_60111A - --Dried food debris was on the food storage containers that were stored as clean. Items removed for cleaning and sanitizing. KDA_4_60111B - --Cooking pans were encrusted with grease and stored as clean. KDA_4_60211E4 - --Dried food debris was on the nozzle of the soda gun behind the bar. Mold was on the walls of the ice bin. KDA_4_90311B - --Utensils were not inverted and stored as clean. KDA_6_30111 - --No hand soap was at the handsink in the food prep area. COS, hand soap provided. KDA_6_50111 - --Broken and missing ceramic tiles throughout the food prep area. KDA_6_501111A - --1 live roach was stuck on glue strap that was on the next to reach in cooler. 1 live roach was crawling on a wall in the back food prep station. 6 drain flies were in the food prep area. PIC stated the last pest control service was 10-21-15. KDA_6_501112 - --Dead roaches were stuck on glue traps that were found behind reach in cooler. KDA_6_50112A - --Dried food debris were under storage rack, warewash equipment, walls in the food prep area. KDA_7_10211 - --A spray bottle with a green liquid was stored on water pipeline. PIC stated the green liquid was a cleaner. COS, labeled. KDA_7_20111A - --A bottle of dish soap was stored next a bottle of Worcestershire sauce on a storage rack. COS, chemical removed. KDA_7_20111B - --A can of stainless steel cleaner was stored on top of single-serve pans. COS, chemical removed.
