The hood vent has an accumulation of grease. The white stack drawer under the counter holding utensils, has an accumulation of dirt and grease on the outside. KDA_4_90311A - --In the basement, there was a box of 1000 count 16/18 oz. drinking cups and a box of solo plastic cups, that were stored on the floor. COS-Person in charge move them to a table. KDA_4_90311B - --The ice bucket at the ice machine, is sitting off the floor with the food contact area face up, where dirt and debris can get in. COS-A clean towel was placed over ice bucket. KDA_5_501113 - --The outdoor trash dumpster had one lid open. COS-lid was shut. KDA_6_20111 - --At the east end of the bar, there is a large piece of flooring missing, leaving a chunk of wood missing in two areas. The flooring in that area, is down to raw wood, making that area hard to clean. KDA_6_30111 - --The hand washing sink did not have soap for hand washing. COS-soap was provided. KDA_6_30114 - --There was not a hand washing sign in the men's restroom. COS-left a sign. KDA_6_50111 - --There is flooring missing through out the establishment. In the women's restroom, there is one stained tile. In the corner by the white reach in, there is ceiling tile that is missing and moved exposing the attic area. KDA_6_50112A - --The shelf under the grill, has an accumulation of grease, food and toothpicks. The south wall behind the bar area, has mud and brown and gray slime around the beer trough. There is dirt and debris under the bar. There is an accumulation of dust on the vent cover above the beer reach in. There are cobwebs hanging down from the ceiling in the game area. The floor in the kitchen area, has an accumulation of grease and debris.
