- 4-102.11(B)(1) - --In two out of the three reach in freezers in the kitchen there was raw meat placed in plastic grocery bags. In make table #3, there was cooked chicken placed in a plastic grocery bag. Corrected on site, items were moved to food safe containers.
- KDA_3_30211A1b - --In the bottom portion of make table #3 there was a metal pan of raw eggs (not in the shell) placed on a shelf above ready to eat egg rolls. Corrected on site, eggs were moved.
- KDA_3_30212 - --On a cart near the grill in the kitchen the following working containers holding food items were not labeled but identified by the person in charge as: sugar, MSG, salt, cooking wine, soy sauce, vinegar and sesame oil. On a shelf below the rice cooker closest to the grill area there was a plastic tub of seasoning salt, made in house, without a label. In the walk in cooler there was a plastic bucket of banana pudding without a label and a metal pan of a brown sauce, made on site, without a label. Near the three vat sink there were plastic tubs of potato starch, corn starch and flour that were not labeled. Corrected on site, items were labeled.
- KDA_3_30412B - --In the kitchen there was a plastic tub of rice with a handled scoop placed in contact with the rice. In the kitchen there were plastic tubs of corn starch and potato starch with non-handled scoops placed in direct contact with the food. Corrected on site, scoops were moved.
- KDA_3_30412C - --On the wall in the kitchen near the prep table there was a magnetic knife strip on the wall that was holding both clean and dirty utensils.
- KDA_3_50113A - --There were packages of frozen meat that had been placed in the three vat sink to thaw. Corrected on site, items were placed in the walk in cooler for thawing.
- KDA_3_50116A1 - --On a cart near the grill area there was a metal container of chicken broth, made in house, at a temperature of 89F. The broth had been out of temperature control since 11 AM that morning. Corrected on site, broth was disposed of down the drain.
- KDA_3_50118A2 - --In the walk in cooler there was a container of pepper chicken without a date mark. Person in charge said that the chicken was cooked yesterday morning. Corrected on site, item was date marked.
- KDA_4_10111A - --In the kitchen near the three vat sink there was flour, corn starch and potato starch stored in plastic containers not bearing a NSF marking.
- KDA_4_30112B - --In the kitchen there was a large pan that was unable to be completely immersed in the three vat sink. Person in charge explained that he puts the pan in the sink sideways and rotates the pan during ware washing.
- KDA_4_60111C - --The range hood had an accumulation of grease above the wok that is most frequently used.
- KDA_4_60211E4 - --The ice chute on the soda fountain machine and the nozzles on the same machine had an accumulation of debris on the food contact surfaces. There was and accumulation of debris on the deflector panel of the ice machine. Education provided.
- KDA_4_90311A - --In the outdoor storage shed there were two boxes of single use cups placed directly on the floor. Corrected on site, items were moved.
- KDA_4_90411A - --On a table near the register there was a plastic tub of single use utensils placed with the food contact surfaces in contact with the handles of other utensils.

- KDA_5_40211 - --There is a direct drain connection at the food prep sink. Person in charge said that food items are placed below the flood rim of the sink. Education provided.
