- KDA_6_501111B4 - --The basement area is a pest harborage area with old furnace, water heater, old reach in cooler, old book, boxes and etc.
- KDA_6_501112 - --A dead mouse was stuck on a glue trap behind the bar. Dead roaches were stuck on glue traps under the bar counter top. Dead roaches were in the cabinet space were single-serve lids were stored.
- KDA_7_20111B - --Cans of inspect spray were stored o shelf above bags of chips (food) in the storage area before going into the basement.
- KDA_7_20612 - --A tray of green rodent pellets was under the 3 compartment sink behind the bar. The rodent bait was not into a bait station.
- KDA_Defacto - --Cooked ground beef, refried beans, and deli ham, deli turkey were in the reach in cooler without opened package date or preparation date. PIC the items were opened and prepared less than 24 hours. --Person in charge provided a test kit for the use of quat.
