- KDA_3_30511A1 - --Boxes of food product were on the floor in the walk in freezer. Boxes of food product was on the floor in the storage area.
- KDA_3_50118A2 - --Pasta salad was stored in WIC without a preparation date. PIC stated the pasta salad was prepared on 10-20-15. COS, pasta salad was dated.
- KDA_4_60111A - --Dried food debris was on the blade of the meat slicer. COS, cleaned and sanitized.
- KDA_4_60111B - --Baking trays and pans were encrusted with grease deposits.
- KDA_4_60111C - --Surface area of cooking equipment were encrusted with grease and food deposits.
- KDA_4_90311A - --Single-serve cups were stored on the floor in the storage area.
- KDA_4_90311B - --Spatulas were not inverted while stored as clean in a plastic storage container. Food storage containers were stacked wet and stored as clean.
- KDA_5_10312 - --The water at the handsink in the warewash area does not have enough water pressure to assure the proper hand washing.
- KDA_5_20515B - --The faucet at the 3 compartment sink leaks water. The water sprayer at the garbage deposal hangs below the flood level.
- KDA_6_30111 - --No hand soap was at a handsink that was located in the warewash area. No hand soap was at the handsink in the food storage area. PIC stated the handsink in the warewash area they do not use that handsink. Explained to the PIC all installed handsink needs to hand soap.
- KDA_6_30112 - --No paper towels were at the handsink in the pizza prep area. COS, paper towels were provided.
- KDA_6_501111B4 - --The broken or non-working equipment throughout food prep area, and food storage areas posses a pest harborage condition. PIC stated pest control provider service date was last week.
- KDA_6_501114A - --A reach in cooler had a out of service sign date of 5/13/13 on the doors. The establishment has multiple reach in coolers, and other non-working restaurant equipment not in working order.
- KDA_6_50112A - --The floor throughout the establishment not was clean. Dried food debris under prep tables, storage racks, and food storage area. Buildup of grease was around the deep fryer area.
- KDA_6_50116 - --A wet mop was left in a mop bucket.
- KDA_6_50119 - --The door of the women's toilet room was propped open during a time when no maintenance operations was taken place.
- KDA_7_20111B - --A spray bottle of white board cleaner was stored in drawer above the prep table. COS, chemical removed.
- KDA_Defacto - --Hot water 138F, handsink. --PIC provided a test kit for the use of quat. --RIC milk 41F RIC cream cheese 39F RIC ham sandwich 39F Salad bar ham 40F, chicken 40F. WIC cooked turkey 38F WIC pasta salad 39F RIC cooked chicken strips 40F RIC raw shell eggs 43F RIC cooked pork sausage 40F There were multiple RIC not in service or working. --Steam table eggs 168F, sausage 177F, gravy 142F
