Observation: Single use spoons were not stored in one direction behind the cashier.

Observation: Several employees were prepping food without hair restraints.

Observation: Utensils were being stored with cleaning equipment in one bucket. The units were moved.

Observation: Several wiping cloths were being left on the counter tops after each use.

Observation: The sanitizer was 0 ppm. The units were remixed and the units were 100 ppm prior to me leaving.

Observation: The facility did not have a written plan for cleaning up diarrhea and vomit. I gave them the state diarrhea vomit clean up guidelines. They stated they would create their own from there.
