    Observation: Food scoop stored in the food pasta in display table while serving the food. Employee removed the and placed cleaned handle up.

    Observation: Wall is peeling away behind the hand washing sink in employee restroom. This is repeat violation.

    Observation: No signed Food Employee Illnesses Reporting Agreement.

    Observation: Fruits was left uncovered in serving line and not protected from contamination. All Food must be covered and protected from contamination when not in immediate use. Cover with plastic wrap.

    Observation: QA sanitzer was too strong at 400ppm in red bucket. Sanitzer concentration must be maintained between 200ppm (neutral). Replace more frequently. Corrected at time of inspection.
  • Package integrity
    Observation: Observed two dented cans of black beans in dry storage. Must discard any can that is dented on top, bottom, or side seals.
  • Handwashing signage
    Observation: No hand washing sign in Employee restrooms. A sign or poster that notifies food employee to wash their hands must be provided at all employee hand washing sink and must be clearly visible to food employees.

    Observation: Wall is peeling away behind the hand washing sink in employee restroom. Re-caulk behind the hand washing sink to provide smooth, nonabsorbent and easily cleanable surface

    Observation: Salsa tomato sauce marked with a date 05/19 that exceeds time in reach in cooler below 41*F. Discard ready to eat food opened from commercial container after 7 days.

    Observation: A plastic white bucket had sanitizer labeled on the bucket which was storing soapy water. The food establishment has RED SANITIZER buckets (pre labeled). The assistant manager labeled the white bucket soapy water during this inspection. CORRECTED ON SITE. --Observation: The sanitizer was over 200 ppm. The water level appeared to be too low. The assistant manager is going to check the numbers which are on the red buckets and tell staff how much water has to be to keep the sanitizer at 200 ppm. This sanitizer was discarded. CORRECTED ON SITE.

    Observation: Several boxes of cereal were stored on the floor in the dry storage area. All food items shall be at least 6" off the floor. The employees placed on the shelves during this inspection. CORRECTED ON SITE.

    Observation: The last inspection was posted BUT NOT VISIBLE due to a large calendar was covering most of the inspection. The most recent inspection shall be posted for consumers to view (in a conspicuous place.)
  • Equipment location, installation, repair, and adjustment
    Observation: 1)In the "freezer room" the Hobart 3 door has torn gaskets. Shall replace to assure the temperature keeps the frozen food frozen. Repair as needed. REPEAT VIOLATION 2) The right side of the Hobart freezer has "heat tape" this is not a smooth, washable, and nonabsorbent surface. Repair as needed.
  • Ventilation hood system and filters designed to be cleanable, prevent dripping, and are adequate to prevent grease or condensation from collecting on walls and ceilings
    Observation: The hood ventilation system has some build up of dust. Assistant manager reported this area is cleaned twice a year. This shall be cleaned as often as necessary to keep clean.

    Observation: The cooling unit in the cafeteria does not have a temperature measuring device. ALL cooling units shall be provided with a thermometer.

    Observation: The butcher block across from the dish machine is peeling. This is a food contact surface which shall be washable, and nonabsorbent. Repair as needed. REPEAT VIOLATION. --Observation: The shelf under the butcher block table has numerous areas where the paint is chipped. Shall resurface this area to be washable and nonabsorbent. Repair as needed.

    Observation: The dry storage area has SEVERAL missing and broken tiles. Repair to provide s smooth, washable surface.

    Floor in dry storage area is no longer smooth, easily cleanable, and nonabsorbent. Some tiles are missing pieces and others are not flush with rest of floor. Must repair or replace tiles to make smooth, easily cleanable, and nonabsobent. REPEAT

    1) Employee RR do not have outside ventilation. RR must be vented to the outside of to remove odors and prevent buildup of bacteria. REPEAT. 2) Fans in kitchen have dust buildup. Must keep clean to prevent buildup of bacteria and contamination.
  • Cutting surfaces maintained
    1) Torn door gasket on Hobart 3 door freezer by the gym (middle door). Must replace gasket to ensure adequate temp is maintained. 2) Butcher block tables (by concession window and opposite diish machine) are no longer smooth, easily cleanable, and nonabsorbent. Tables have deep scores, pits, and cracks. Tables must be resurfaced or replaced to prevent buildup of bacteria and contamination.
