Observation: Fan covers and ceilings in the walk in coolers are soiled with a large accumulation of dust. Floors around equipment and fixtures are soiled in the deli/meat department.

Observation: Tenderizer blades soiled with food debris. Tenderizer was soaked and thoroughly cleaned and sanitized at time of inspection.

Observation: Vegetable dip packaged on site for consumer with a sell by date of 5/4/15 on display in produce dept. Dip was discarded voluntarily.

Observation: Shelves used to stored single use items in the meat department are soiled.

Observation: Raw ground beef stored over whole muscle cuts in the meat cooler. Ground beef moved to a lower shelf.

Observation: Dish soaps displayed over single use items (sandwich bags and aluminum foil) on the store shelves. Single use items moved to a higher shelf for display.

Observation: Cutting board surfaces in the meat department are worn with numerous deep, discolored scratches.

Observation: Two light bulbs in the kitchen/meat department are not shielded.
- Cleanability of floors, walls, and ceilings
Observation: Wall by the warewashing sink in the deli/meat department is losing it's smooth, cleanable coating exposing the fiberboard material underneath. Doors to the walk in coolers and freezer have a number of punctures with jagged edges in them.
