Observation: 1) Raw chicken ,2) beef patties-ground beef, 3) pork lions meat, 4) pork lemon were stored above ready to eat food (RTE) in the reach- in display store cooler for sale. Must store by the cook temperature- higher cook temperature items must be stored on lower shelf. Removed and corrected on site.
- Light intensity
Observation: Missing light or bulbs burned out in the following:1) Nesquick milk reach-in cooler, 2) Butter and yogurt cooler. Light bulbs must be replaced in areas where there is exposed food. Replace.
- Storage or display of food in contact with water or ice
Observation: Boxes of food stored in walk-in freezer exposed to leaking water under lines and must be removed to prevent contamination with water or ice. Removed and corrected on site. --Observation: Ice buildup in chest freezer "Bunny" and "Italian ice- cream" chest freezer in the store. Must defrost to prevent source of contamination. Must keep freezer free of ice.
- License required to operate a food establishment
Observation: Most recent license was not posted for consumers to view.License was in the manager office upstairs. Must post the most recent inspection report in area easily readable by the public. Corrected on site.
- Heating, ventilating, and air conditioning systems design, installation, and cleaning
Observation: Air conditioning system and vents are not working in walk-in freezer and started dripping on the floor. Must repair. --Observation: Vets are dusty, dust is build up in walk-in cooler. Clean as often as necessary to keep clean and prevent contamination.
- Package integrity
Observation: Several dented can food such as, diced tomato, chicken with rice, stored on dry goods shelf and should be removed or discarded. Removed and corrected on site.

Observation:1) Gaskets on last 5 doors on right side of beverages reach in cooler have gaskets that are torn and no longer easily cleanable, replace. 2)Has scales for weighing meats that does not have a current sticker of inspection by state, contact Weights and Measures at 515-725-1492 for inspection.

Observation:Has gap in lower edge of double doors at back of building, seal where can see light showing through.

Observation:Has outdated inspection report posted, must post most recent inspection.

- Food storage - preventing contamination from the premises
In walk in cooler has two bags of ice and several cases of frozen foods stored on floor, must store all foods at least 6 inches off floor to protect from contamination.
- Outer openings are protected
Gaps under or around exterior door at lift area in back dry storage area, seal where can see light showing through.
- equipment food contact surfaces and utensils clean to sight and touch.
Has one area of soiled shelving from beverage spill in walk in beverage cooler, may need to reseal shelving if finish has worn off, to make all surfaces smooth, cleanable and nonabsorbent holding food products, including beverages.
